The post holder will work within their clinical competence as part of a multi-disciplinary team to: provide expertise in clinical medicines management provide structured medication reviews with and for patients support patients in the management of their long-term conditions oversee/support the management of medicines on transfer of care and implement systems for safer prescribing manage repeat prescription authorisations and acute prescription requests.

The post holder will perform medication reviews for patients with polypharmacy, especially older people, people in residential care homes and those with multiple co-morbidities; this may be in a practice setting or via phone or video consultation as agreed with the practice and the Lead Clinical Pharmacist.
The post holder will be involved in quality improvement and clinical audit as well as liaising with practices to manage some aspects of Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) work. This role is pivotal to improving the quality of patient care and to developing operational efficiencies; it requires motivation and passion to deliver an excellent service within general practice.
JOB RESPONSIBILITIES Manage a medicines management plan and deliver patient services as determined by and with the Lead Clinical Pharmacist, practice / PCN policy and local and national guidance. Work as part of the medicines management team within the PCN/practices and with medicines management colleagues within the ICB to acquire and share expertise and best practice to maximise cost-effective prescribing and improve the quality of patient care.

Engagement engage with PCN Clinical Pharmacist colleagues and the CD responsible for Clinical Pharmacy to contribute to the development of the PCN medicines management approach and team perform clinical audits around key areas of responsibility Patient services provide medication review services to patients via clinics in member practices, domiciliary visits and in residential and nursing homes, either face-to face or via remote consultation technology and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care liaise with the PCN Education and Environment Lead (EEL) to arrange presentations at practice and patient group meetings or other events to give advice on the appropriate use of medicines.
