To provide personal care such as feeding, toileting assistance to individuals that need it, and will undertake manual handling duties as required
To assist with facilitating sessions in line with group and individual EHCP outcomes

Experience of supporting people to achieve outcomes through SMART processes
Duration: Initially 3 months Hours: 28.8 hours per week term time only Rate: £20 umbrella an hour £16 PAYE an hour
Southwark Council are looking for a Support Assistant to join their All Age Disability Team. As a Support Assistant you will:
Support the delivery of a service that achieves positive outcomes for young people, facilitating maximum independence, dignity, choice and control, through person centred planning and supportTo provide personal care such as feeding, toileting assistance to individuals that need it, and will undertake manual handling duties as required
To assist with facilitating sessions in line with group and individual EHCP outcomes
To maintain up to date and accurate record, including both electronic and paper systems
Requirements
Knowledge of working with young people with SEND needs, or vulnerable adults including either holding a Care Certificate or a willingness to undertakeWorking knowledge of alternative communication systems
Experience of working with young people with SEND needs or vulnerable adults, or within a PBS service
Experience of supporting people to achieve outcomes through SMART processes
Experience of implementing plans and supporting people through crisis
