Key Responsibilities: Patient Care: Provide urgent, out-of-hospital care to patients as part of the PCN, assessing and managing a wide range of clinical conditions, delivering high-quality care to patients in their homes or in the GP practice.
Collaborative Working: Work closely with GPs, nurses, care coordinators, clinical pharmacists, physiotherapists, and other healthcare professionals within the PCN to ensure the delivery of integrated, patient-centered care. Multidisciplinary Team Support: Work as part of a multidisciplinary team, to manage patients with acute concerns, complex health needs, frailty, and long-term conditions.
Health Promotion and Prevention: Support proactive and preventative care, including identifying health risks, conducting health assessments, and offering advice on lifestyle changes to reduce future health issues. Health Inequalities & Population Health: Contribute to reducing health inequalities and ensuring that services meet local population health needs, particularly for vulnerable groups and those with chronic conditions.
Clinical Support: Provide on-the-spot clinical assessments, treatment, and referrals for patients as required, ensuring they receive the right care at the right time. Documentation & Reporting: Maintain accurate, detailed records of patient care and interventions in line with NHS and PCN standards.
