Are you an experienced band 5 Occupational Therapist looking for an opportunity to step up into a band 6 position in Stroke or an existing band 6 looking for a change? If so an exciting opportunity has opened up for a band 6 specialist Occupational Therapist to join the South Nottingham Integrated Community Stroke Service (ICSS).

This post is a 37.5 hrs permanent position.

Our established ICSS delivers rehabilitation into various community settings, currently between 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future.

The team offers full or part time roles, including flexible working which aims to achieve the right work-life balance, maintaining the wellbeing of our workforce.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust.

This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.

Main duties of the job

This role requires day-to-day delivery of client care in our Integrated Community Stroke Service.

To use specialist clinical reasoning, critical thinking, reflection and analysis to support assessments, treatment and management of stroke survivors with complex presentations. To work closely with the MDT, communicate effectively with clients/carers and their families and supervise, teach and develop other staff as required.

Support our patients within the community to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke.

Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings.

Detailed job description and main responsibilities

We are looking for a Specialist Occupational Therapist to join our dedicated Community Stroke Team across the South of Nottinghamshire.

This post will allow the successful applicant the opportunity to work within an integrated multi-disciplinary team, providing therapy to clients referred to the Community Stroke Rehabilitation services, including early support stroke rehabilitation.

The multidisciplinary team includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Clinical Neuro-Psychology, Assistant Practitioners and Rehabilitation support workers. We also provide a post stroke review for clients at 6 and 12 months.

As a Community Stroke service, we aim to offer a high quality therapy led Multidisciplinary Team approach providing stroke rehabilitation in a patient’s home or care home, focused around achievement of agreed rehabilitation goals.

As an Occupational therapist in the Community Stroke team you would be expected to
  • To provide high quality holistic evidence based care to patients / service users that is responsive, effective and efficient.
  • To facilitate and support timely discharge and prevent inappropriate admissions by supporting patients / service users to remain as independent as possible and maintain or improve their level of function and quality of life.
  • To be professionally and legally accountable as an autonomous professional for all aspects of own work and delegate to staff, students and others.
  • To formulate individualised, innovative management and treatment plans using clinical reasoning, critical thinking, reflection, analysis and evidence based practise.
  • To manage a caseload using specialist Occupational Therapy assessment for stroke clients who may have complex and diverse presentations, and plan appropriate management and treatment using specialist clinical reasoning skills and a wide range of therapeutic skills
  • To work closely with the other members of the multi- disciplinary team within health and outside agencies to ensure management and treatment meets the holistic needs of the client
  • To support quality and service improvement, by involvement in clinical audit.
  • To providing effective clinical supervision and assist with caseload monitoring of less experienced staff
  • To participate in the Trust clinical supervision and PAD appraisal systems carrying out supervision and PAD reviews as delegated, reporting any issues of concern to the Team lead
  • To supervise, teach and develop colleagues within area of expertise using evidence based practice where possible.
  • To support the organisation and delivery of in-service training and initiate informal training sessions when required to ensure high standards of clinical care.
  • To complete clinical records to Trust and HCPC standards
  • To maintain own CPD, keeping up to date with clinical developments and current research and evidence based practice
  • To provide clinical education of Occupational Therapy students in liaison with universities to ensure expected quality standards are met

The Community Stroke Team is split into three localities. The base of this role is within in the locality of Nottingham West. The team work closely and will require you to work flexibly, across the whole of South Nottinghamshire, to support our evolving and developing service, supporting the team to deliver high quality integrated care.

This position requires you to work independently, but also be a team player, working closely with our Integrated Care Provider partners providing care to a diverse population covering many different geographies including, Broxtowe, Gedling and Rushcliffe.
