Job overview

We are recruiting for a Macmillan Personalised Care Support Worker who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

This is an exciting opportunity to join the cancer outpatient team at Lister Hospital, in a brand new role. You will work closely with cancer patients to identify social and support needs through the completion of electronic holistic needs assessments (eHNA).
Using the eHNA tool to identify any areas of concern for the patient and signpost as needed, facilitating personalisation of patient care. You will also assist with end of treatment summaries (EOTS) utilising the Macmillan My Care Plan eHNA system to improve patient experience.

The role will include liaising with community and social services and working with the existing community link workers to ensure a seamless service across the cancer centre, particularly for vulnerable patients.

As part of the multi -disciplinary team (MDT) you will contribute to service provision by providing elements of direct and indirect patient care and clerical work. You will be expected to facilitate and support care delivery, information giving and communication and coordination.

Main duties of the job

To support patients to complete electronic holistic needs assessments, identify concerns and support the implementation of the care plan.

To work closely with staff across the patients cancer journey and within the community to support the patients social and support needs

Receive patient calls and make pre planned outbound calls to patients to assess ongoing neds.

To support vulnerable patients and those with complex support and care requirements who are being treated at LMCC.

Skills

Essential criteria
  • Able to demonstrate a successful track record of developing own clinical practice.
  • Good communication skills
  • Effective inter-personal skills.
  • Ability to work as a member of a team & autonomously.
  • Able to convey empathy to patients and carers.
Desirable criteria
  • Variety of teaching skills experience
  • Experience of providing education to all levels of staff and patients.

Qualifications / Training

Essential criteria
  • Comprehensive secondary education with a minimum of 4 GCSE’s or equivalent.
  • Further educations particularly within healthcare.
  • Experience at band 3.
  • Cancer patient assessment experience, using a trust recognised tool.
  • NVQ level 3 or 4
Desirable criteria
  • Willingness to undertake further training as required.

Previous Experience

Essential criteria
  • Experience of working with haematology or solid oncology patients.
  • Able to work across organisational boundaries including with primary care.
  • Able to use problem solving techniques and escalate rapidly to senior staff.
  • Able to work productively without supervision.
  • Willingness to assisting in audit and research.
Desirable criteria
  • Experience of working with cancer patients.
