The City and Hackney Place Based Partnership is seeking a Director to lead its development and drive delivery of its strategic objectives
This is an exciting role charged with leading local health and care system partners into an accelerated phase of care transformation, integration, and service improvement for the residents of the City of London and Hackney.
The City and Hackney Place-based partnership is a collaborative that comprises the City of London Corporation, the London Borough of Hackney, Homerton Healthcare FT, East London NHS FT, City and Hackney GP Confederation, the eight Primary Care Networks, representation from the C&H Charity and Voluntary Sector Leadership Group, along with lay and clinical leaders, which takes responsibility for:- Working with local residents and communities to improve their health and wellbeing,
- addressing longstanding inequalities, and
- providing high quality, integrated and sustainable health and care
The PBP Director of Partnerships, Impact and Delivery will be responsible for supporting the Place-based partnership to deliver a focused programme of work to improve the health and wellbeing outcomes of people living in City and Hackney. This will cover short-term interventions as well as the delivery of longer- term strategic priorities.
The post holder will do this by ensuring that local resources – including the care professional workforce – are deployed strategically and coherently in response to local peoples’ needs. The roles will also require the postholder to play a key part in programmes of work across NEL ICS led by NEL ICB.
The City and Hackney PBP is one of seven PBPs within the North East London Integrated Care System and reflects the ambition of NEL ICS to delegate resources to place partnerships to ensure we make a reality of joined-up and integrated care which responds to the specific health and care needs of local communities.
The City and Hackney PBP Director of Partnership, Impact and Delivery will be employed by Homerton Healthcare but will work on behalf of the local partnership and will be responsible to the City and Hackney Health and Care Board and accountable to the PBP Place Leader.The post holder will also be a director within NEL ICB and will have accountability to the ICB for delivery of its objectives at place and across the system.
