Job overview

An exciting opportunity has arisen to join the outreach and care home liaison team. The teams are merging into one to deliver a 7 day service to support Older people with a mental health diagnosis in care homes and people living with dementia either in their own homes or within care homes and in crisis.

The aim is to prevent admissions and support discharges.

The service is evolving and developing and we are loving for a dynamic and creative individual to join this team. There are plenty of opportunities to develop skills and training is available through a robust CPD scheme.

Main duties of the job

A key part of this role is to support the care home sector and community mental health teams in preventing inappropriate admissions to hospital by supporting the sector particularly on an evening/weekend.

As a member of multi-agency team, under direct and indirect supervision, to assist qualified staff. Will assist staff with assessment, provision and evaluation of care and treatment to patients, predominantly over the age of 65 suffering from mental health/Dementia problems in a community or inpatient setting.

To undertake a range of delegated duties including: assisting in offering practical advice, support and guidance to clients, carers and their families, as appropriate to their situation, as part of an agreed care plan under the direction of one or more team members.
