Assistant Care Manager – Carers team - Oakham

An exciting opportunity has arisen for an Assistant Care Manager to join Rutland County Council’s Carers team. You will be liaising with GPs, District Nurses, case managers, primary health practitioners, statutory and voluntary agencies regarding the best possible care to support the carer in their caring role.

If you have experience of working within Adult Social Care and undertaking social care assessments by providing outcome focused support plans, we want to hear from you.

About you…
  • Maths & English GCSE Grade C or above, or equivalent.
  • Relevant experience of working within a health, social care or voluntary sector
setting.
  • Working knowledge of people with care and support needs.
  • Knowledge of relevant legislation and recent developments in social care.
  • Knowledge/experience of safeguarding adults.
  • Experience of maintaining effective and confidential records.
  • Able to communicate with others in a positive and respectful manner.

About the role…

The Assistant Care Manager will undertake the following:
  • Identify carers and provide advice and information to support them to continue
in their caring role and ensure the identified outcomes for carers are met.
  • Support requests for a carers assessment under the Care Act.
  • Ensure carers have a contingency plan in place should the carer be unable to
continue in their caring role, whether in an emergency or planned cease of
their caring role.
  • Assess and complete health and social care assessments.
  • Provide assessment, evidence-based intervention, practical advice and
emotional support for family carers at all stages, being responsive in periods of

loss and change.

This role will work as part of a multidisciplinary, integrated team to support Rutland residents or those registered with a Rutland GP, to co-produce a simple personalised care and support plan to address the person’s health and wellbeing needs by introducing or reconnecting people to community groups and statutory services to enable them to continue to be a carer should they wish to do so.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.

The Assistant Care Manager interviews will be held on 4 November 2024.
