JOB DESCRIPTION Post:
  • Associate Clinical Nurse Specialist (Neurovascular) band 6 Line Manager: Clinical Nurse Specialist/ Neurovascular Advanced Nurse Practitioner (ANP) Principal Accountabilities Assess, plan, implement and evaluate programmes of evidence
  • based care for the neurovascular in and outpatient.

Provide expert advice and support in clinical practice.

Participate in the education and training of other staff members Be a role model for all staff through provision of high standards of professional practice Work collaboratively and co-operatively with the multi-disciplinary team to develop services and deliver high quality evidenced based care to patients within the inpatient and outpatient settings.

Promote standards through audit/service review Clinical Perform comprehensive assessment of patient; plan, implement and evaluate care delivery according to changing health care needs, seeking support where necessary.
Collect, collate, evaluate and report information, maintaining accurate patient records Involve patients/carers and relatives in the planning and delivery of care.
Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to ongoing care needs and discharge arrangements.
Establish and maintain effective communication with patients, carers/relatives and professionals across health and social services.
Evaluate effectiveness of interventions and change accordingly.
Perform advanced clinical skills, and demonstrate specialist knowledge of neurovascular conditions, which is underpinned by evidence as appropriate to level of experience.
Identify and organise appropriate investigations where necessary.
Pre-admission assessment of the neurovascular patient.
Leadership Provide specialist, clinical knowledge to clinical colleagues, and support to patients and carers/relatives.
Participate in nurse-led clinics to follow up and provide people with information and support regarding their condition with support from medical team/ CNS/ANP.

Participate in the development of services and assist CNS/ANP to implement changes in service delivery Develop skills and advance practice using the clinical supervision framework with appropriate support from the Neurosurgical Nurse Specialist team.

Participate with links in the community to promote and increase awareness of the services provided to those patients who have a diagnosis of subarachnoid haemorrhage.
Participate in the planning and delivery of an effective neurovascular service Ensure the effective and efficient use of physical and financial resources.
Monitor health, safety and security of self and others and promote best practice in the clinical area.
Clinical Governance Continually monitor standards of care and participate in the improvement of care, through benchmarking, audit and research.
Ensure compliance with policies, procedures and clinical guidelines for self and others.
Promote patient and public involvement activities in the specialist area, leading to service improvement.
Investigate critical incidents and service failures with assistance if required.
Practice safely and effectively within scope of knowledge and skills, seeking support from team members appropriately.
Understand and implement safeguarding where needed.
Accurate documentation and appropriate information to sources within and outside the hospital to uphold continuing care.
Involvement in multidisciplinary team meetings to promote safe effective care.
Be involved in ongoing audit and research of neurovascular patients.
Education and Development Provide education and development opportunities for others Develop own expert clinical knowledge and skills and participate in the development of others clinical knowledge.
Acquire skills for effective and safe clinical examination.
Promote reflective practice by facilitating discussion about good practice and problem areas within the clinical supervision framework.
Provide information, education and support to recently diagnosed patients and their families.

Maintain up to date skills and knowledge and maintain awareness of professional issues Attend annual appraisal with the ANP to identify and agree training and development needs as part of Personal Development Plans Maintain registration and comply with NMC professional code of conduct Ensure own compliance and compliance of others with regard to mandatory training and revalidation requirements Attend training sessions to build upon existing clinical skills to further enhance clinical practice.
