We are seeking well-motivated and forward-thinking Registered Nurses with an interest in End of Life Care to join our RCN award-winning multi-disciplinary team on our In-Patient unit. The right person for this role will have a passion for working collaboratively in an environment thriving on positive change and making a difference to those with palliative and end-of-life care needs, including patients, families and colleagues alike.
As a Registered Nurse you, will join our RCN award-winning multi-disciplinary team on our In-Patient unit and you will : Plan, implement and evaluate evidence-based nursing care strategies for patients and their carers in an open and cooperative manner respecting the rights of the individual.
Anticipate and react to the changing needs of patients and emotionally prepared the patient and family. Participate in MDT discussions to inform the management plan for patients. Work In conjunction with the band sixes, lead on the day-to-day management of the discharge process.
Disseminate the management plans for the patients to the rest of the nursing team. Manage the admission of patients. Complete the verification of the death process when a patient dies.
