Job overview

An exciting opportunity has arisen to appoint a full time Band 6 Dietitian in Home Enteral Feeding to work within Aneurin Bevan University Health Board as part of the Community Dietetic Team.

We are seeking an experienced, highly motivated and enthusiastic Dietitian to join our friendly, forward thinking and dynamic Team. Within this role you will develop your skills and knowledge in Home Enteral Feeding, to deliver and monitor a home enteral feeding service as part of a specialist team, supporting a complex and varied caseload of adults living with tube feeding.

The post-holder will have experience of assessing, monitoring and supporting individuals with enteral tube feeding / community dietetics and will have a compassionate and flexible approach to providing patient centred care.

You will be based at the offices in Ty Gwent in Cwmbran with the wider Community Dietetics team.

Dietetic experience is required in the relevant area, an Annex 21 contract for Band 5 to 6 roles can be considered.

Main duties of the job

The post-holder will be responsible for supporting the clinical care of a caseload of adult patients living with home enteral feeding across ABUHB / Gwent.

The role supports patients in managing their individualised tube feeding regimens at home and ensuring enteral feed deliveries are maintained safely. The service supports patient care remotely and delivers direct patient facing care during home visits.

You will need to be highly organised, flexible and work well under pressure to support patients in managing their tube feeding plans at home & support changes to their tube feeding regimens as required.

You will be required to nutritionally diagnose, and formulate treatment plans in consultation with patients, relatives and carers, to provide patient centred care..

You will be working alongside the community MDTs – including GPs, District Nurses, Speech Therapists, Occupational therapists etc; to contribute to the assessment and care need of individuals.

You will be expected to travel independently across all localities within ABUHB to visit patients homes and attend appropriate MDTs when required.

There will be opportunities to contribute to service innovation and support national research. To provide training for dietetic colleagues and other health professionals with respect to nutrition and HEF and supporting their clinical practice.

The post holder will also be required to support the community dietetics team with a generic case load.
