Case Manager - Bristol
We’re a small, experienced and supportive team looking for a Case Manager to join us at Blue Skies Case Management & Care. Our workload is diverse and we support both adults and children with complex needs. Our role is to provide a rehabilitation package that maximises the potential of the client, keeps them safe and allows them to live their lives to the fullest.
As a Case Manager, you’ll have the opportunity to manage your own caseload, coordinate essential services and advocate for your clients, all while being part of a collaborative and friendly team. No two days are the same, and you’ll be working alongside professionals who are passionate about delivering high-quality, person-centred care.
We welcome experienced Case Managers as well as those new to the role as we provide mentoring, training and ongoing support to help you thrive. Key Responsibilities:- Conduct comprehensive assessments of clients' needs, strengths, and preferences.
- Develop and implement individualised care plans in collaboration with clients, families and healthcare providers.
- Coordinate and monitor the delivery of services, including medical care, social services, housing and financial assistance.
- Advocate for clients' rights and needs, ensuring they receive appropriate and timely services.
- Maintain accurate and up-to-date client records, documentation and reports.
- Provide ongoing support, counselling and crisis intervention as needed.
- Facilitate communication between clients, families, healthcare providers and community resources.
- Participate in multidisciplinary team meetings and case conferences to ensure holistic care.
- Stay informed about changes in regulations, policies and best practices in case management and social services.
- Health Care professionals with a minimum of 8 years' experience in their current profession. Case management experience preferable but not essential as training will be given.
- Case managers to be registered member with BABICM or CMSUK.
- Strong knowledge of community resources, healthcare systems and social service programmes.
- Excellent interpersonal, organisational and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records (EHR) and case management software.
- Valid driving licence and reliable transportation for client visits.
