East Dorset ICRT are looking for Band 3 Community Rehabilitation Assistants to work within the Multidisciplinary team, based at St Leonards Community Hospital, MDT working as a team to provide intermediate care and rehabilitation to the adult population of the East Dorset Locality.

We are a friendly and dynamic team, that are committed to providing high quality Rehabilitation and care to the residents of East Dorset through integrated and collaborative ways of working in conjunction with our colleagues in the Primary and Social Care sectors.

The post will involve treatment of patients in their own homes to avoid hospital admission and enable them to achieve as much strength, confidence, function, and independence as possible following their illness/surgery. We also support patients who wish to remain at home who are requiring End of Life Care.

Main duties of the job

The Integrated Community Rehabilitation Team is a multi-disciplinary team offering therapy to a wide range of people within their home environments to prevent avoidable hospital admissions and facilitate earlier hospital discharge as well as in-reaching onto the wards.
It is essential that you have experience of rehabilitation or a caring role, preferably in both ward and community settings, as well as having good verbal, written and team working skills. You will need to be flexible in your approach and confident to work alone.

Basic IT skills and NVQ level 2/3 would be advantageous.

Responsibilities will require you to work mainly under the close supervision of the clinicians within the team, but at times also to work autonomously. You will need to have the ability to drive a car as you will be required to visit patients in their own homes and other community settings.

In return for your enthusiasm and commitment, we can provide an opportunity for you to develop your clinical skills and experience in a supportive team, with regular supervision and appraisals. You will be required to achieve a certain competency level which we will give you support and training to help you reach.

Opportunities for further training may be available through the Trusts Training Department.

May work alone within scope of role and have their own caseload of service users with non-complex needs that have been previously seen or triaged by a registered member of staff.

EDUCATION, QUALIFICATIONS & TRAINING

Essential criteria
  • NVQ level 2/3 or equivalent award in an appropriate field
  • Demonstrable competence in skills for role

EXPERIENCE

Essential criteria
  • Experience in a care based workplace
  • Experience in care of the older person
  • Demonstrate enthusiasm for providing high standards of patient care
Desirable criteria
  • Working within the NHS

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria
  • Basic computer skills for example entering numbers in a spreadsheet or using email
  • Ability to communicate effectively with patients/clients despite barriers to understanding that enables them to participate in their therapy
  • Ability to work autonomously and manage workload

TRANSPORT

  • Car owner
  • Full UK Driving Licence
