We are seeking a dedicated and compassionate Social Worker to join our team on a permanent basis. This role is critical in providing support and intervention to individuals and families in need of assistance, helping them navigate through personal challenges and improve their overall well-being.
As a Social Worker, you will work in various settings including schools, healthcare facilities, and community organizations to assess clients' needs, develop tailored intervention plans, and implement therapeutic interventions. Our ideal candidate will be skilled in building rapport with clients from diverse backgrounds and will have a strong understanding of social services and community resources available.
Your role will also involve collaborating with multidisciplinary teams, advocating for vulnerable populations, and ensuring that clients have access to vital services. Additionally, you will be tasked with monitoring client progress and making necessary adjustments to support them effectively.

Responsibilities
Conduct assessments to evaluate clients' needs and strengths.
Develop and implement treatment plans tailored to individual client goals.
Provide counseling and support to clients and their families.
Collaborate with healthcare providers, schools, and community organizations to coordinate services.
Advocate for clients to ensure they receive necessary resources and support.
Document client progress and update case records in compliance with organizational standards.

Participate in team meetings and case conferences to discuss client care strategies.

Requirements
Bachelor's or Master's degree in Social Work from an accredited institution.
Valid state licensure or certification as a Social Worker.
Strong understanding of social service programs and community resources.
Excellent communication and interpersonal skills.
Ability to work effectively with diverse populations and build rapport.
Proficient in case management and client documentation processes.

Strong problem-solving skills and ability to handle crisis situations.

Hours Per Week: 36.00

Start Time: 09:00
End Time: 17:00
Pay Per Hour: 08HC £35,235 - £39,513 (plus market supplement of £5,000 per year)
Relocation package of up to £10,000 available

Location: Hereford, Herefordshire
