This is a permanent community nursing and care coordinator role within the diverse Stepney and Wapping Neighborhood Mental Health Team.

We are looking for an enthusiastic and skilled Community Mental Health Nurse Band 6 with an interest in working in Stepney and Wapping Neighbourhood Mental Health Team that has a positive approach to supporting service users with their recovery.

The Tower Hamlets Stepney and Wapping Neighbourhood Mental Health Team is a well-established team within East London Foundation Trust and operates as an integrated multi-disciplinary team with staff from both the East London NHS Foundation Trust and the London Borough of Tower Hamlets working together to provide a high-quality service for people with severe and enduring mental health needs this is inline with Transformation Project.

The team actively works in partnership with service users, carers, the Local Authority and other statutory services and local voluntary groups.

To promote the needs of service users with mental health needs and to offer and provide training and education whenever possible. In conjunction with the crisis pathway services, provide a safe and effective urgent community assessment and treatment response service.Undertake comprehensive assessments, including risk assessment, effectively and efficiently and undertake Care Act assessment to facilitate social care service and safeguarding enquiry when required.

Develop and maintain a high standard of practice based on assessment through the use of psychoeducation and the Recovery Model. Liaise and establish good working relationships with referring agents, and colleagues in Primary Care services, Adult social care and other agencies as appropriate.
To know of and practice a range of therapeutic interventions available to reduce distress for individuals experiencing an acute mental health crisis. Ability to work autonomously and within an MDT setting.To work with the multi-disciplinary team in developing plans that offer supportive crisis care in the least restrictive way.
Act as a role model in the practice area, encouraging staff in teaching and supervision to sustain a supportive learning environment. Be responsible for the maintenance and accurate recording of patient information in the documentation systems promptly and to ensure the effective dissemination of service user information throughout the multidisciplinary team.
Ensure compliance with the requirements of Safeguarding Adults and Children’s policies. Have a good understanding of Care Act Assessment and carer assessment. Have an understanding of the KPIs for the crisis service and ensure compliance with Trust CQUINs.

Ensure your compliance with mandatory training is maintained. Ensure full potential through supervision, appraisals and performance management. Ensure that you are also suitably skilled in addressing the physical health needs of service users.
