We are pleased to offer an exciting opportunity for a Nurse or AHP to apply for a band 7 Diabetes Team lead within North Cumbria Integrated Care Trust
This is a fantastic opportunity for a dedicated and enthusiastic person who is passionate about Diabetes to utilise their clinical and leadership skills while supporting the development of the Service across North Cumbria
The post holder will be an integral member of the Diabetes Service specialist team, providing leadership and line management to a specialist team of professionals and also care, treatment and education to diabetes patients and their families.This will involve working closely with the community network of health care professionals to deliver an integrated package of care between hospital and primary care. In conjunction with the Lead clinician and Service Manager the Team Lead is accountable for the quality of diabetes Services provided by the Trust.
The post holder will be based in Carlisle however will be required at times to travel across North Cumbria.
The post is for 30 hours per week.
Main duties of the job- Provide clinical input, demonstrating advanced competence in complex decision making, assessment and management of clinical needs of patients with Diabetes and often complex needs.
- Actively manage and supporting the development of individuals and the team through appraisal, personal development planning, coaching and mentoring
- Provide professional assurance and quality governance in the service
- Leading on the implementation of clinical protocols/pathways in identified service lines
- Further develop rehabilitation and recovery focused services to enable more people to be independent for longer, manage their health needs and lead healthy, active lives in their community with a key focus on Diabetes.
- Supporting the service in its implementation of clinical effectiveness programs by taking part in any agreed research and audit projects, leading on key aspects when identified as appropriate
- Provide strategic clinical leadership using the most up to date and evidence based theory
- Offer expert clinical advice for service users, carers, staff and other professional
- Make a contribution to training, teaching and supervision of colleagues
- Work in partnership with service users, carers and other agencies to ensure effectiveness of outcomes

Person specification
Qualifications
Essential criteria- Degree level education (Nursing/AHP)
- NMC or AHP professional registration
- NST307 or equivalent
- Post-Basic qualification in specialist area (Diabetes e.g. Warwick Course A205, CIDC, Bradford Diabetes course) or will work towards within the first year of appointment
- Adult Education Teaching Certificate
- To have or be willing to undertake a diabetes related qualification such as Warwick or Bradford
Knowledge
Essential criteria- General IT skills including experience of windows based software and MS Office
- Understanding of current NHS issues and initiatives
- Knowledge of clinical governance and its application
- Up to date knowledge of the developments in diabetes care
Skills & Aptitudes
Essential criteria- Demonstrable clinical skills within specialist area, including physical assessment skills and basic life support
- Ability to obtain and evaluate information to aid decision making.
- Confident, diplomatic, good organisational ability, pleasant disposition, able to work in isolation but part of a team, co-operative & flexible, able to deal with difficult/emotional situations
Personal Circumstances & Other requirements
Essential criteria- Willing to undertake training as required.
- To able to travel independently of public transport and be able to travel widely as and when required.
- Physically fit to stand for long periods, manoeuvre wheelchairs, and assist patients.
- Criminal record check satisfactory to the organisation
Experience
Essential criteria- Significant general nursing/AHP experience within the NHS
- To have detailed knowledge of all aspects of diabetes and associated risk factors.
- To have the ability to work autonomously, efficiently and professionally locality wide visiting patients in General Practice surgeries, Health centres, community and acute hospitals and emergency home visits.
- Experience of teaching adults
- To have or be willing to become a DESMOND/ DAFNE/ Walking Away trainer
- Experience of undertaking clinical audit
