Community Staff Nurse, Young Greenwich School Nursing

Key Task and Responsibilities The Young Greenwich Community Staff Nurse will be part of the wider nursing team in delivering identified aspects of the Healthy Child Programme: To work with parents/carers, children, other professionals including education and statutory/voluntary organisations.
To participate in the delivery of Young Greenwich service key touch points. These include promoting the annual year 6 health & wellbeing reviews. To plan, deliver and evaluate health promotion work with school aged children, families and the wider Young Greenwich community as per service offer.
To facilitate teaching sessions for school staff about recognising and responding to conditions including Anaphylaxis, Sickle Cell Anaemia, Asthma and Epilepsy. To deliver health and wellbeing support drop-ins and groups for families held within the community hubs using the Solihull Approach.
Responsible for ordering and maintaining health promotion stock and additional resources required for service delivery. Act as the Duty Nurse managing incoming direct referrals from young people, parents/carers, schools and other professionals to ensure needs are responded to as quickly as possible.
To work in partnership with Greenwich childrens social care by appropriately sharing health information and attending safeguarding meetings including child protection conferences, core groups and team around the child/family meetings where an identified health need requires a health intervention.
Be proactive in the development, review and promotion of the Young Greenwich service. To maintain awareness and adherence to Oxleas NHS Foundation Trust policies and procedures and Young Greenwich standards. To support schools to achieve Healthy Schools accreditation and participation of the SHEU survey biannually.
Support national and local health promotion campaigns whilst working with stakeholders and the wider Young Greenwich team. Demonstrates a clear understanding of own responsibilities and contributes to team working and development of partnership initiatives.
To work according to the NMC code, 6 Cs of nursing, Oxleas NHS Foundation Trust Nursing Strategy and the ethos and aims of the Young Greenwich Partnership to ensure high quality, responsive patient focused care is delivered to all children and families.
Management and Leadership responsibilities To plan own workload effectively to meet client needs, Young Greenwich activity targets and key touch points in accordance to the service offer. Participate in staff meetings / forums / specialist forums and working parties contributing to service development and maintain own professional knowledge.
To participate in supervision (management, safeguarding and clinical) and the PDR processes for self and supervisees, ensuring that supervision and PDR time requirements are met. To support the orientation programme for new staff and support junior members of the team.
To contribute and participate in the conduciveness of a learning environment which leads to an effective placement experience for under and post graduate nurses on community placement. To support the Youre Welcome accreditation for YG hubs. Clinical Ensure own mandatory training is kept up to date in line with Trust guidelines.
To deliver the key touch points as per the Young Greenwich Partnership and 5-19 Healthy child programme. To formulate and review care plans and medical alert booklets with parents, schools and other agencies for children with specific medical conditions or on-going health concerns.
Support and advise schools / Families / GPs in relation to medicine management and the safe storage of emergency medication in schools highlighting expiry dates/ dosage requirements when required. To be an autonomous practitioner and follow the lone working policy to maintain safety.
Research Contribute to service development ensuring that all care is evidence based, and implementing agreed changes. Contribute to Audit processes and the implementation of updated practices. To be proactive in ensuring where appropriate and able to do so a patient experience / service evaluation is captured.
Communication and Record Keeping Establish and maintain good relationships with clients, carers and the wider partnership team. Communicate effectively with schools and parents in relation to the YG registration process, consent and YG referral pathway.
To interpret sensitive information and communicate with a sympathetic and empathic approach to client centred care. Ensure effective verbal and written communication is maintained within the team at all times to ensure timely, effective and coordinated care for families.
Maintain contemporaneous and accurate records daily on all active clients according to with record keeping standards, policy and NMC code using manual and electronic forms of recording including RiO diary outcomes for confirmation of contacts.
Maintain IT systems such as RiO or CRM for the CYP registration, tracking, referrals and communicating across the partnership and to support reporting for key touch points. Ensure correct use of audiometers, scales and height metres, the calibration requirements of the equipment and report any deficiencies to PA or lead nurse.
Responsible for the correct safe use of IT equipment such as laptop and reports defects to IT or Lead Nurse. To acknowledge that all work undertaken is of a highly confidential nature and therefore the need to observe and maintain this confidentiality is essential: general understanding of GDPR and process for reporting any data breaches in line with Trust policy.
Complete and submit financial claims on a monthly basis within the deadline. To inform line manager of workload pressures that may adversely affect the level and quality of service provided. On Call/Unsocial Hours Core hours are Monday to Friday 9am-5pm.

There may be occasional requirement to contribute to extended service hours of 8 am - 8 pm or weekend health promotion events.
