Providing dietetic assessment, advice and care planning to community patients, Working alongside other health professionals to provide a MDT approach to the care of vulnerable elderly patients with nutritional needs.

Work with HEF patients to ensure their care is safe and effective

Qualifications and Requirements:

Hold a registration with HCPC
Fully enhanced valid DBS

Dietry expereicne required.
