Job overview

We are seeking an experienced and compassionate Advanced Nurse Practitioner to join our community-based Treatment Team.

As part of our commitment to improving patient care and outcomes, we are establishing a new treatment team specifically focused on supporting patients receiving depot medication. This is an exciting opportunity to be at the forefront of developing and implementing a vital service that will have a lasting impact on the community.

As the ANP lead in the Community Mental Health Treatment Team working as an advanced practitioner in the community setting, You will work collaboratively in providing flexible, dynamic and innovative care, responsive to health needs, focusing on those service users who present with mental health needs and on depot medication.

Main duties of the job

As an experienced Advanced Nurse Practitioner take responsibility for the development, planning and implementation of care and delivering treatment for those patients requiring medication advice and review, and those with wider complex presentations

Lead the setup of the new treatment team, including mentoring nursing and support staff. Foster a collaborative and supportive team culture focused on high standards of care

To manage a case load of service users who have complex health and care needs and who require medication advice and/or review in a defined time period of support.

Work in partnership with the GP and wider Secondary and Primary Health Care Team to ensure seamless care and integrated care across pathways

Provide expert clinical oversight for the administration of depot injections, ensuring all procedures adhere to best practice guidelines and regulatory standards.

Qualifications

Essential criteria
  • Current NMC Registration
  • RN 1st Level
  • Evidence of continuing professional development
Desirable criteria
  • Mental Health Nurse Qualification
  • To be educated to Masters level or working towards.
  • Undertaken Advanced Nurse Practitioner course

Training

Essential criteria
  • Experience of the teaching/mentoring students
  • Able to deliver talks/training to small groups

Experience

Essential criteria
  • Experience of care co – ordination/care management
  • Experience of instigating investigations, interpreting investigations and initiating appropriate plan of care for patients with Mental Ill health
Desirable criteria
  • Experience of working in the Community Mental Health care setting
