Practice Nurse

Service Care Solutions are looking for a full-time Practice Nurse to support a GP Surgery in Bradford, on a locum basis.

System: S1

Pay Rate: £28 LTD p/h

Responsibilities:

• Ensure accurate and timely recording of data and updating of care plans using SystmOne.
• Responsible for the assessment, planning, implementation, and evaluation of evidence-based individualised programmes of care.
• Co-ordinate, manage, and organise the nursing caseload.
• Recognise changes in patients' conditions and refer to other professionals when necessary.
• Provide care for patients with long-term conditions.
• Community visits required

Qualifications and Requirements:

• Fully enhanced valid DBS.
• Must have previous community experience.
• Long term conditions Nurse needed (LTC)
• Must be able to offer community aswell - £0.59p per mile but need business insurance
