Transfer of Care Pathway Facilitator
To respond to telephone queries from referring clinicians in acutes/CCGs/HUBS/Primary care about their patients regarding status of referrals To participate in regular audits necessary to monitor effectiveness of service provision and identify data issues across each care pathway.
To be aware of status of acute and community facilities on a daily basis through relevant dashboard list and ensure all members of the team are informed of any updates. Carry out information gathering functions on a daily basis, enabling efficient provision of service and supporting the role of the service Uphold patient confidentiality at all times and adhere to NHS guidelines Actively input into the development and provision of alternative pathways To assist in maintaining links with Acutes/ Intermediate Care Hubs /Primary Care / CCG colleagues about patients regarding status of referrals etc Effectively communicate with relevant agencies across the whole system including TOC / LUHFT Co-Ordinators, Intermediate Care Teams and Urgent Care Teams to collate and transfer accurate information, relating to service availability and pressures,
