CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
Due to maternity leave, we have an opportunity for a Senior Nurse or Paramedic to join the team on a fixed term basis. We are looking for forward thinking, passionate staff who can take a positive risk management approach to support the frail population to remain at home when acutely unwell.
The Hospital at Home model has been developed with our system partners WHTH and provide face to face care to facilitate acutely unwell patients out of hospital to home quicker or by preventing hospital admissions.
This service expands upon of our existing VH models within the system and has been implemented in line with National Guidance. The team is clinically led by a Frailty Nurse Consultant and the service delivered by ACPs, ECPs, senior nurses and therapists and support workers.The Hospital at Home service sits within the Unplanned Care Business Unit and will align with CLCH’s Urgent Community Response service and cover the four localities in West Hertfordshire.
Main duties of the job
We welcome applications from a nursing or paramedic background to build upon the existing ECP team. We anticipate that successful applicants will be a current Non-Medical Prescriber who is competent in prescribing for older people and have a good knowledge base of medications.
System working with our acute hospitals will be a key aspect to the delivery of this service which will generate the opportunity for you to develop skills in system working and service development.

Person specification
Education/Qualifications
Essential criteria- Registered Nurse with current NMC registration or Registered Paramedic) with HCPC.
- Evidence of continued professional development.
- Independent Prescriber with an up to date relevant scope of practice.
- Qualification in Top to Toe assessment
- Qualification in teaching or practice-based learning.
- University accredited Minor illness Module
- Qualification in prehospital care or community care
Experience
Essential criteria- Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.
- Experience of effectively working in collaboration, negotiating and liaising across organisational boundaries.
- Experience in monitoring clinical standards.
- Proven ability to prioritise and manage own workload with minimal supervision.
- Experience caring for frail patients
- Experience of team / service/ staff management.
- Experience of implementing evidence-based care and developing others to do so
- Experience of conducting research projects and audit.
- Experience of change management and implementing change in practice.
- Evidence of innovation in practice.
Skills and Knowledge
Essential criteria- Evidence of comprehensive range of clinical skills and expertise relevant to the role
- Ability to communicate effectively both verbally and in writing
- Basic IT skills including use of Microsoft Office package and email
- Specialist knowledge in clinical reasoning, history taking, physical examination, diagnostic skills and prescribing/treatment.
- Advanced knowledge and understanding of anatomy and physiology of body systems.
- Clinical knowledge across a range of long-term conditions.
- Knowledge of NMC/ HCPC Code of Professional Conduct.
- Knowledge of current health care policy and legislation and relevance to the delivery of healthcare services
- Detailed knowledge of Primary Health Care and collaborative working with other care providers, e.g. social care and voluntary services
