We have an excellent opportunity for a part time senior clinical pharmacist to join our mental health pharmacy team. You will work in a small team based on service need. Areas include the in-patient, home treatment team, community mental health team and living well units with no dispensary or out of hours commitment providing the perfect opportunity for you to develop your clinical skills and broaden your mental health knowledge and experience.

To provide, manage, organise and develop the clinical pharmacy service to the allocated area of Pennine Care NHS Foundation Trust to cover for gaps in senior leadership

To ensure that the clinical pharmacy service provided meets with the national standards for the delivery of the pharmaceutical services to mental health.

To ensure that all developments required for the pharmacy service are implemented and learning spread trust wide

To ensure that pharmacy services work within agreed guidelines to ensure effective medicines management

To ensure the delivery of effective medicines management in conjunction with medical and nursing staff.

To ensure the implementation of all Pennine Care NHS Foundation Trust policies and procedures that impact on medicines management and pharmaceutical care.

To work in close liaison with other pharmacists and technical staff providing a pharmacy service to Pennine Care NHS Foundation Trust under Service Level Agreement (SLA), to ensure the delivery and supply of medication to the mental health wards, departments and units.

To work with the Chief Pharmacist to ensure that there is liaison with the Service Directors and/ or medical manager about medicines management issues, as necessary.

To provide cover for other pharmacists providing a service to and employed by Pennine Care NHS Foundation Trust, as requested.

You will be supported by a lively clinical pharmacy team who are dedicated to delivering excellent services. You will receive clinical and and management supervision and have opportunities to develop yourself and the role through education, training, team meetings and opportunities such as Quality Improvement and audit.

You will be supported to undertake further qualifications as needed such as non medical prescribing. The role is entirely clinical with no dispensary, out of hours or on call commitment

To act as the specialist mental health pharmacist for the clinical pharmacy service to the allocated area including mental health wards, departments and home treatment teams.

To conduct clinical pharmacy visits to the allocated PCFT area.

To review prescriptions to ensure safe and effective prescribing and appropriate to the individual patients and to recommend the monitoring of mental and physical health, as necessary.

To attend multi-disciplinary team meetings to advise on matters relating to choice and use of medicines and carry out any necessary preparatory work to maximise pharmaceutical input.

To prepare drug history reports for patients detailing positive, negative and adverse responses to past treatment and making recommendations for future treatment strategies.

To act as an expert source of pharmaceutical advice for health and social care staff, patients and carers and provide a clinical pharmaceutical service when medical information is conflicting

To answer queries about medication from other healthcare professionals using critical appraisal skills so that the information provided is evidence based.

To ensure that the medication provided to the mental health wards and units are stored appropriately to main quality and potency.

To ensure that any concerns in prescribing practice are highlighted to the South network lead pharmacist so that appropriate action can be taken.

To provide advice to medical, nursing and other healthcare professionals on the actions and properties of drugs and on their safe and effective use, ensuring an evidence-based approach.

To handle urgent clinical queries and prioritise tasks according to the level of urgency.

To monitor ward drug usage.

To promote economical drug usage.

To participate in and oversee the development of clinical audit both within the pharmaceutical service and as part of multi-disciplinary audit.

To ensure that medication error reports are reported via the Ulysses Risk Management System.

To ensure that any trends in relation to medication errors are brought to the attention of the network lead pharmacist and support the role of the medication safety officer

To ensure that any information required by staff working within the mental health wards and units on medicines is provided and that the resources are available to provide the information.
