We are currently looking for a occupational therapist to work across our pathway between SPOC / In-reach team and our rehabilitation beds.
The post holder will have access to multi-organisational partners to improve patient experience and will support patient flow to contribute to timely and excellent standards of care. Working across the pathway will give the post holder significant exposure and experience to the workings of the NHS system.
If you have experience of working in acute, community or rehab services dealing with complex challenges and are keen to do something different in a highly performing, stimulating and fast-paced environment then this may be your opportunity.
You will be fully supported by the team and service manager as you grow your knowledge of clinical services within West Hertfordshire and beyond to ensure patients receive the most appropriate care in a timely manner
Main duties of the job
Work in partnership between primary and secondary care, assist with the development, implementation and evaluation of a discharge planning service, for patients that are being discharged from acute hospital services into the care of community-based services.
Assist the discharge planning teams in acute hospitals to ensure that service users are referred to the most suitable community pathway. Where disagreement exits, to work collaboratively to ensure the best outcome for the patient.

Person specification
Education/Qualifications
Essential criteria- Degree in Occupational Therapy Registered Occupational Therapist
- Current HCPC registration
- Demonstratable successful innovation in clinical practice
- Evidence of CPD relevant to acute and rehabilitation practice
- Up to date CPD portfolio
- Evidence of service improvement within previous role
- Member of BAOT
- Diploma in Management Studies or equivalent / Post graduate qualification in specialist area.
Experience
Essential criteria- Previous experience working within either an acute hospital or rehab unit or community service
- Able to communicate with patients, relatives and MDT and on occasions deal with conflicting views
- Appropriate clinical experience post registration to work at senior level
- Experience of an working as an OT in at least acute / rehabilitation
- Experience working in multi-disciplinary team
- NHS experience
- Knowledge of common causes of delays which lead to a delayed transfer of care (DTOC) for patients
Skills and knowledge
Essential criteria- Knowledge of the local health and social care system
- Knowledge of NHS reforms and key local and national policy drivers
- Knowledge of acute hospital settings and where common delays to the transfer of care may occur
- Advanced skills in assessment and treatment of people with multi-pathologies
- Ability to apply core OT skills across a range of clinical conditions with client centred, goal setting approach.
- Communicate effectively at all levels
- Computer literacy
- Well-developed verbal and written communication skills
- Knowledge of community-based nursing and therapy pathways and rehab units

Essential criteria- Car driver – with access to car insured for business – unless Equality Act (2010) applies.
- To participate in 7 day working – weekend and bank holidays included.
