PURPOSE OF THE ROLE: The post holder will play a key role in supporting the Continuing Healthcare (CHC) Service. Working within the remit of the current National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, the post holder will deliver an effective and competent level of service that is consistently client-focused.

The post holder will assist and support in the facilitation of multi-disciplinary assessments and reviews, in line with standard operating policy and procedures. The post holder should possess basic knowledge of the NHS Framework for Continuing Healthcare and NHS Funded Nursing Care; along with local policy and practice related to CHC.K EY RESPONSIBILTIES OF THE ROLE: Information Management Responsibilities, including: Supporting the Continuing Healthcare team to ensure National Framework compliant processes and local processes are completed in a timely manner, as well as suggesting amendments and or new processes are implemented to support the effective running of the service.

Communicating information, risks, issues and dependencies, including briefings and reports. Policy and Service Development: Actively support the Continuing Healthcare team in working towards goals and objectives. Maintaining up-to-date knowledge of legislation, policies and guidance.
Support the delivery and development of standard operating procedures and processes, including those for Personal Health Budgets (PHBs), within the team. Ensuring the principles and duties of safeguarding are holistically, consistently and continuously applied.
Makes suggestion on improving their own working area to improve service delivery. Human Resources Shows junior staff and peers support with their local inductions. Operational Support: Highly motivated and enthusiastic individual to provide and coordinate administrative services to include sourcing and collating records, populating national tools for CHC, the preparation of agendas and minutes, taking appropriate follow-up action as required.
The post holder will be required to review and collate patient related documentation of individuals who are requesting NHS Continuing Health Care funding or require a Review of their care needs. On occasions, this may involve travelling to an individuals care setting.
The post holder will be required to identify evidence of health care needs by following the guidelines as described in the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care using various computer systems and liaising with other health and social care professionals.
Use up to date knowledge and evidence when undertaking care reviews and care planning, advising and promoting health and well-being The post holder will make person centered, evidence-based judgements in partnership with others involved in the care process to ensure high quality care.
Recognise the limits of own competency and appropriately request supervision from a Registered Nurse to ensure the delivery of commissioned safe, high-quality care. Deliver and coordinate clinical reviews, personalised health budgets and care planning to a caseload of patients as delegated by the Line Manager.
Ensure that all interventions are patient centered; delivered compassionately, with dignity; whilst challenging and escalating poor practice / performance. Report any concerns related to patients care following case management to a Registered Nurse within the Continuing Healthcare Team or Continuing Care Team for appropriate escalation to Care Quality Commission, Safeguarding services.
Communication and Relationships: Excellent communication skills to enable effective communication with a range of internal and external stakeholders. Communicate clearly, effectively and confidently with people in the workplace, following detailed and/or multi-step instructions.
Provide written and oral information on specific projects/workstreams. Produce effective emails, short reports, presenting information in a manner suitable for the audience. Work with others towards achieving shared goals, learning from mistakes and being open to the opinions of others, receiving and giving constructive feedback.
Demonstrate honesty, integrity, care and compassion when dealing with others ensuring you utilise tact and persuasion skills as and when necessary. Assess the root cause of the problems, and deal with them and with people with honesty and integrity, showing interest in your work and seeking and valuing the contributions of others.
Understand your rights and responsibilities in the workplace including promoting equality of opportunity, valuing diversity and maintaining confidentiality. Make best use of resources including time, to achieve agreed goals for service delivery, taking responsibility for own actions and self-development and demonstrating effective team leadership.
Be open and respond constructively to change, coping with uncertainty and assessing and minimising risk. To arrange / schedule assessments and reviews for CHC Assessors, including contacting individuals and family members / representatives and other relevant professionals, as required.
The post holder will be required to record formal minutes for a range of meetings where discussions are often complex and detailed. Recording subsequent decisions made and ensuring appropriate follow up action is taken to progress these decisions.
Support and maintain appropriate and effective office management systems to ensure a smooth flow of information within the department. Working proactively, implementing changes to local procedures and processes to improve efficiency, where appropriate.
Collaborative Working and Stakeholder Management: Maintaining constructive relationships with a broad range of internal and external stakeholders. Working closely with care providers involved in delivery of the service. Fostering transparency and a person-centered approach with individuals, families and representatives.
Leadership and Staff Management: Attending team meetings including team supervision sessions for support; identifying and sharing good practice. Act as a point of contact for the department, dealing with routine and specialist enquiries in a pleasant and helpful manner, communicating relevant information to stakeholders, referring to others as appropriate.
Able to work on own initiative and unsupervised, within defined policies and procedures to set timescales. The post holder will carry out any other delegated duties that may be required by a manager. The post holder will always adhere to the service guidance, and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed.
Financial and Physical Resources: To work within agreed systems, processes and to attend specialist panels to secure best value for money, and packages of care that meet an individuals assessed clinical needs. To ensure case management represents value for money within available funding.
Planning and Organisation: Managing time effectively, prioritising work to meet competing demands and deadlines and adjusting these as priorities change. The postholder is expected to be self-motivated and self-starting to be capable of acting on own initiative in areas such as background research and problem solving, as well as deciding when it is necessary to refer to line management.

Processing high volumes of information quickly and accurately. Ability to deal with sensitive and confidential issues in a fashion that merits empathy, tact, and professionalism.
