Job overview

The role of Discharge coordinator is an integral part of the MDT providing support and specialist advice to the clinical nursing team, AHPs, medical teams and extended MDT including social care and at times outside agencies. You will be expected to provide the highest level of support and advice to enable the timely discharge of vulnerable and graduate premature infants to home and to the care of their family/carers and evaluating our services.

The post holder will use their experience and knowledge to support transition from hospital to home, working closely with community partners ensuring a safe and supportive environment is achieved with ongoing and continued support is available where necessary.

You will also be expected to actively contribute to multi professional and multi agency meetings being able to give reports, information and updates to the relevant parties including safeguarding meetings for which there is a link role between the safeguarding team and the neonatal unit.

Safeguarding updates and pathways already in place within the trust will need to be imbedded in daily practice with focus on pathways particular to the neonatal ward to be strengthened. The post holder will be key for leading on quality and safety initiatives which will include the setting and monitoring of standards with the direction and support of the Matron.

Main duties of the job

We have an opportunity for a Band 7 Neonatal Discharge Coordinator at 0.5 WTE which has become available in Neonatal ICU , Royal London Hospital to join our dynamic and friendly team. We are looking for a motivated , ambitious, and experienced Neonatal Nurse who will support and work closely with the Neonatal team , working in partnership with families in the unit to ensure seamless transition of care from hospital to community by liaising with the Neonatal Community outreach team , pertinent agencies within and outside agencies within and outside the organisation.

Qualifications

Essential criteria
  • Registered Nurse, Children’s Nurse/ Adult Nurse
  • Mentorship and assessment in public services.
  • Neonatal course (all modules)
Desirable criteria
  • BLS for parents - Train the trainer
  • NLS

Experience

Essential criteria
  • Proven managerial and professional experience at band 6 level with good experience of discharge planning for babies.
  • Neonatal experience at staff nurse level in intensive care or local neonatal unit.
  • Proven professional competence.
  • Awareness of research and audit
  • Evidence of continuing education in relevant field of practice.
Desirable criteria
  • Counselling skills/courses

Knowledge

Essential criteria
  • Up to date knowledge of professional development
  • Knowledge of NMC working codes for medications, documentation, conduct and professionalism.
  • Good knowledge of childrens safeguarding processes and pathways.
  • Knowledge of complaints procedure and data protection.
Desirable criteria
  • Experience of working with social services, social workers, and various disability and community support services.

Skills

Essential criteria
  • Ability to prioritise well with good problem solving skills.
  • Excellent communication skills both verbal and written.
  • Good level of skill in computer usage – able to use NHS data bases, input data and retrieve information.
  • Awareness of current issues and reforms in the NHS and neonatal services.
  • Awareness of the Toolkit for high quality neonatal services
  • Excellent understanding of safeguarding issues.
Desirable criteria
  • Proven skills at working at senior nurse level within a level 3 neonatal unit.
  • Shows desire to implement new ideas and improvements.
