Job Description 1. Job Details Job Title: School Nurse Staff Nurse - Band 5 Service : School Nursing Targeted Intervention Service Midlands Partnership University Foundation NHS Trust Department/Directorate Children and Families Reports to (Title): School Nurse Team Leader Accountable to (Title): Childrens Services Lead 2.
Job Purpose/Main Duties/Responsibilities The post holder will be responsible for their own caseload within their speciality area as an autonomous practitioner. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care to a specialist level ensuring that they practice within their sphere of competence and knowledge.
The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge. 3. Nature of the Service The School Health Service is integral to the delivery of the Healthy Child Programme; it is the universal prevention and early intervention service for school age children that will enable them to achieve their optimum health and wellbeing.
The primary aim of the service is to achieve the best health and wellbeing outcomes for school age children through the healthy child programme of health and development reviews, immunisations, health promotion and interventions tailored to individual risks and protective factors.
The School Nursing service is undergoing significant change through Service Review and future changes to commissioning arrangements. 4. Organisation Chart Service Lead Team Leader School Nurse( Band 6) School Nurse (Band 5, this post) Support Staff Key Responsibilities/Duties To work across the City as a member of the School Health team, participating in activities which address the health needs of school aged children and young people.
Focussing specifically on children with complex needs in our special educational schools. To contribute to the identification of the Public Health needs of individuals and communities within the defined client group; using evolving school profiles of health needs; and working within specified agreed national and local targets.
To plan negotiate and carry out initial and any subsequent on-going child health assessments for specific caseload, using relevant policies, procedures and guidelines; and make appropriate referrals to health and multi-agency partners as required.
To assess, plan and support children and young people with regards to all continence issues, including nocturnal enuresis, toileting programmes and assessing if there is a requirement for containment products. Subsequently assessing and ordering appropriate products if deemed necessary.
To develop, implement and evaluate individual care plans for children with specific health needs To adhere to Staffordshire Safe Guarding Children Boards guidelines, to identify and report any case of suspected or confirmed abuse. To manage any identified health needs and support any children and their families within the Child Protection and Looked After system To attend and participate in Case Conferences, Reviews and Core Group Meetings as required.
To write and supply reports in accordance with Child Protection policies including writing reports for and attend Court if necessary To act as an advocate for children and young people with behaviour and mental health needs, offering independent Tier 1 (1st level) intervention.
To work in collaboration with Child and Adolescent Mental Health (CAMHs) staff and make referrals as appropriate To undertake any training and development necessary to fulfil the requirements of the post. To maintain professional development through attending appropriate multidisciplinary forums, conferences, seminars and training courses.
To contribute to the development and implementation of policies across the Trust reflecting service developments and improvements. Responsibilities of the Post Holder for Specific Areas To occasionally be, the initial point of contact in identifying and reporting any case of suspected or confirmed child abuse The ability to manage child protection issues in a safe and consistent manner, which will include attendance at case conferences, reviews and core group meetings.
There is a need to remain objective whilst sharing sensitive information with the possibility that this may cause antagonism between parents and professionals. On average this may occur once a week To work in situations that are unpredictable, lone working, home visits and responding to client demands.
Occasional exposure to verbal aggression. Freedom to Act To be responsible for making a professional judgement and acting on the correct interpretation of Fraser Guidelines, (a means of deeming whether a young person under the legal age of consent is competent to consent to treatment, without parental consent) in a variety of settings e.g. schools and clinics Initiates action to identify and minimise risk and ensure child protection safety, working in partnership with parents.

This will involve working closely with Social Services and attendance at core groups and case conferences. Supervision will be provided by senior team members To be responsible for making professionally informed decisions and judgements, of a sensitive and emotive nature, which may be challenged by other professionals and parent/carers on a daily basis To act as an advocate for children with behaviour and mental health needs, to work in collaboration with Child and Adolescent Mental Health (CAMHs) staff and make referrals as appropriate To accept and act on referrals from agencies such as education staff, family doctors, CAMHs staff, health visitors, social services, community and acute based paediatricians, voluntary groups, parent/carers and young people 8 Effort & Environment Factors Physical demands of the job Daily driving on a number of occasions per shift in busy built up areas, accessing and parking at school premises, clinics etc Daily lifting, handling and transporting of equipment and records of moderate weight in and out of health centre premises, car boots and school buildings Regular use of computer keyboard skills To work in confined spaces, poor environmental conditions such as corridors, halls and cloakrooms with school Systems and Equipment To use screening equipment on a daily basis, for example, Audiometer, Leicester Height Measure, weighing scales, vision charts and ensure it is calibrated and maintained and used correctly To adhere to Trust Patient Group Directives To be responsible for inputting personally generated clinical data on computer system or in patient records To maintain contemporaneous, accurate records as required by professional code of conduct and Trust guidelines
