Specialist Nurse (Hospital Discharge Team)
You will have experience of working with patients in both acute and community settings, as well as experience of discharging patients from an acute hospital setting; however, if you are confident, innovative and have a broad range of clinical experience to draw on these would also be valuable skills.
The role will be varied and is expected to develop over the coming months, as the newly formed team adapts to a new model of working and seeks to improve the discharge process for those patients requiring some form of care when they leave hospital.
Previous experience in assessing, planning and reviewing care is essential. An understanding of the range of care available to patients to both avoid hospital admission and to support hospital discharge is also essential. The post holder will also be expected to identify, record and to work collaboratively to resolve any delays associated with discharge from an acute hospital, therefore understanding the Pathways for discharge from hospital and how to access them is also essential.
We are a friendly and supportive team, who work closely with colleagues from all areas of Health and Social Care, both acute and community based.
