Responsibilities Based in the Clinical Nurse Specialist team, you will have a balance of clinical and managerial responsibilities. Supporting the Lead Nurse with the day-to-day management of the service, deputising in their absence. Managing the service efficiently and effectively.
This can include clinical assessments, triaging, care planning, clinical supervision, mentoring, and assessing workload / service activity. A corporate caseload management framework is required Completing the rota for the team, ensuring adequate cover & skill mix Assessing the needs of the patients and families on caseloads and co ordinate appropriately.
Undertaking appraisals of team members as directed by the Lead Nurse Supporting with staff members' revalidation as a requirement for practice and maintaining NMC registration. Providing supervision to the team, e.g. 1:1 sessions with line manager, clinical supervision, group/peer supervision Attending internal and external meetings as a representative of St Margarets Hospice Care, as directed by the Lead Nurse Community Services.
This will involve cross county working. Acting as a role model to the community team, providing both clinical and leadership responsibilities. Leading and engaging the team through change and development Identifying areas of learning and utilising training opportunities.
Keeping updated with organisational policies and procedures. Undertaking the holistic assessment, planning and delivery of a consistently high standard of specialist palliative and supportive care. Acting as a specialist practitioner, managing a caseload of patients with advanced, progressive life limiting diseases Establishing effective working relationships with wider multi-disciplinary teams.
