We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it.

Our community services include health visiting, school nursing and support for families of children with long-term conditions.

Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

Job overview

The GSTT Post ICU Recovery Clinic is a comprehensive post ICU recovery service founded in 2016 that has already supported >3700 complex critically ill patients and their families beyond ICU and the first year of their discharge home. Patients receive full assessment of their physical, cognitive and psychological function through multidisciplinary assessments by consultant, critical care nurse, occupational therapist, physiotherapist, clinical psychologist and neuropsychiatrist.

This Clinical Fellow post in Recovery after Critical Illness offers an exciting and unique opportunity to gain clinical experience in critical care aftercare under direct consultant supervision. You will contribute to the delivery of post ICU Recovery Services of a varied and challenging case mix (including the COVID19 and SRF/ECMO cohort).
Training will be provided in multi-professional aspects of recovery after critical illness across the recovery continuum spanning both inpatient and outpatient phases. Additional experience will be available in our parallel aftercare programmes including: the ICU survivor peer support group, the patient diary project, psychological interventions, supporting driving resumption after ICU.

An innovative digital recovery platform enabling patient-centred care and rehabilitation is also under evaluation. Opportunities are anticipated for quality improvement and research into outcomes after critical illness.

Main duties of the job

This post will combine clinical duties in the Post ICU Recovery Clinic (50%) and direct clinical care duties in the critical care department (50%).
  • Full training in all aspects of post critical illness follow-up will be provided. This will include assessment of post intensive care syndrome, multi-disciplinary interventions to promote recovery, patient- and family-centred outcomes from critical illness, health related quality of life instruments, and social/financial aspects of recovery. Exposure to peer support, patient diaries, patient experience and ICU discharge coordination is also provided.
  • Duties will include, but are not limited to:
  • Screening and telephone pre-assessment of potential clinic patients
  • Delivery of critical illness recovery clinic and assessment of PICS under consultant supervision
  • Compose clinic letters and coordinate onward referrals
  • Maintenance and development of follow-up database and contribution to ongoing audit/research activities
  • Contribute to development and expansion of follow-up services to suit needs of GSTT case mix
  • Quality improvement of our service including information and resources for patients and families
  • Feedback and dissemination of follow-up activities to the wider critical care department
  • Attendance at relevant meetings
  • Teaching and training of medical, nursing and allied staff as well as medical students

Detailed job description and main responsibilities

Clinical:

The postholder will, together with colleagues, be responsible for the provision of services to the Guy’s & St Thomas’ NHS Foundation Trust to include:

(a) Diagnosis and treatment of patients of the trust in such hospitals, health centres or clinics or other premises as required.

(b) Continuing clinical responsibility for the patients in your charge, allowing for all proper delegation to, and training of, your staff.

Training of junior staff:

The postholder will take responsibility for the training and direction of the multi-disciplinary staff allocated to him/her under the aegis of the training plan that that postholder has agreed with their supervising consultant.

Clinical Governance:

All medical and dental staff are expected to take part in clinical governance activity, including clinical audit, clinical guideline and protocol development and clinical risk management. They will be expected to produce evidence of their contribution in these areas and their audit of their own clinical work as part of their appraisal.

Qualifications / Education

Essential criteria
  • MBBS or equivalent medical qualification
  • FRAC/MRCP/MRCS or equivalent qualification
Desirable criteria
  • DICM/EDIC or equivalent qualification in intensive care medicine

Professional / Statutory Registration

Essential criteria
  • Eligible for full registration with licence to practice with the GMC at time of appointment
  • Eligible to work in the UK

Previous Experience

Essential criteria
  • Ability to provide a complete employment history with no unexplained career gaps
  • Evidence that career progression is consistent with personal circumstances
  • Evidence that present achievement and performance is commensurate with totality of period of training
  • Minimum 18 months substantive experience in intensive care medicine
Desirable criteria
  • Experience delivering follow up care to patients and families after critical illness
  • Knowledge of ICU survivorship and outcomes
  • Demonstrable research experience
  • Demonstrable teaching experience

Skills / Knowledge / Ability

Essential criteria
  • Valid adult life support (ALS) certification
  • Airway skills (minimum 12 months substantive anaesthesia)
  • Excellent verbal and written communication skills for a wide range of audiences
  • Excellent interpersonal skills, including confident and comfortable interaction with multi-disciplinary colleagues
Desirable criteria
  • 24 months substantive experience in anaesthesia
  • Fluent use of IT including email, word processing, spreadsheet, database
  • Expertise in statistics and data analysis
