Job overview

The Community Response Team supports discharges from the acute hospitals, providing short-term and interim care.

Previous caring experience is ESSENTIAL.

Your base will be Babington Hospital to cover the localities of Amber Valley and Erewash.

You will need to be flexible and committed to caring and supporting people at home with care needs.

You will need to be able to travel between visits, therefore a full UK driving licence and access to a vehicle is essential.

Shifts follow a 7 day rota, including weekends and bank holidays.

Main duties of the job
  • To be responsible for the day to day supervision of Support Workers within the Community Response Team.
  • Delegate patient visits to ensure an effective support worker service is provided to patients in a timely and cost effective way.
  • To be a role model to the Support Worker team and actively monitor and maintain clinical skills and professional manner.
  • Communicate effectively with patients and carers to maximise rehabilitation potential as appropriate and under supervision.
  • Work with qualified Nurses and Therapists in the assessment and treatment of patients with a variety of conditions and progress/modify intervention programmes within parameters set by the Therapists and Nurses.

Education Qualifications and training

Essential criteria
  • NVQ level 4 or equivalent knowledge gained through experience and training
  • GCSE English and Maths
  • Experience working as a band 3 Support Worker
  • Community experience
  • Experience working with adults/older people in rehab setting

Skills and attributes

Essential criteria
  • Car driver or ability to travel between caseloads as part of daily role
  • Manual handling skills and experience

Aptitude and personal qualities

Essential criteria
  • Ability to work flexibly to meet needs of service
  • Ability to travel within wide geographical area (AV and ERE)
