  • non-clinical HOURS 25 hours /week Job summary The purpose of the role is to Take a lead role in all aspects of care for diabetic patients Provide routine primary care nursing services for practice patients, to include blood tests, dressings, injections, assist with minor surgery, vaccinations etc To provide patients with health promotion and ill health prevention advice in accordance with NICE guidance and practice protocols To assist the GPs with procedures such as minor surgery and chaperoning To assist in the healthcare and support provided to patients with chronic diseases, specifically in relation to those conditions covered by the QOF criteria To share the responsibility for the management of the clinical environment within the practice, including stock control, cold chain management, infection control, maintaining equipment etc Duties and responsibilities: The duties and responsibilities to be undertaken by members of the practice nursing team may include any or all of the items in the following list.
This list is not exhaustive and duties may be varied from time to time under the direction of the GPs, Lead Nurse and Practice Manager, dependent on current and evolving practice workload and staffing levels Providing ongoing care and support to diabetic patients and adopt a lead role in the practice in relation to these patients Wound care, ulcer care, Doppler Removal of sutures New patient health checks Travel advice and vaccinations ECGs, d-dimer tests Annual reviews for chronic diseases such as asthma, diabetes, COPD, hypertension Venepuncture, including anticoagulation monitoring
  • INR-Star Childhood vaccinations Requesting pathology tests, including urine, swabs, blood Maintenance and cleaning of equipment used by the nursing team and GPs Assisting with stock control Use of the EMIS computer system to create and maintain accurate, contemporaneous and appropriate clinical records Management of tasks, emails etc, with appropriate response times and actions Audits Review and development of protocols Ensuring up-to-date knowledge of NICE guidelines and PGDs Participation in clinical governance and other practice meetings Administrative tasks associated with the nursing role Communication The post-holder should recognise the importance of effective communication within the team and will strive to Communicate effectively with other team members Communicate effectively with patients and carers Recognise peoples needs for alternative methods of communication and respond accordingly Health & safety The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice health and safety policy, the practice health and safety manual, and the practice infection control policy and published procedures.
This will include Using personal security systems within the workplace according to practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Actively reporting of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general, patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role Undertaking periodic infection control training
  • minimum annually Reporting potential risks identified Quality The post-holder will strive to maintain quality within the practice, and will Alert other team members to issues of quality and risk Enable patients to access appropriate professions in the team Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Know the practice policies which are available in the practice staff handbook Record all information relating to patients, carers etc, in an accurate and timely fashion, using manual or computer systems as appropriate Take the necessary precautions when transmitting information Contribution to the implementation of services The post-holder will Apply practice policies, standards and guidance Discuss with other members of the team how the policies, standards and guidelines will affect own work Participate in audit where appropriate Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.

They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers.

They may also have access to information relating to the practice as a business organisation.

All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and protection of personal and sensitive data Personal/professional development This post is subject to a probation period of at least 6 months.

Probation review meetings will be undertaken by the Senior Practice Nurse at 1, 3 and 6 months.
The practice may decide to extend the probation period if further training is needed to undertake the full role outlined above, or there are concerns regarding the performance of the individual which need to be addressed.
The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include on-site training, e-learning and off-site training as appropriate.
They will participate in an annual individual performance review, including taking responsibility for maintaining a record of their own persona and/or professional development.

It should therefore be assumed that this list is not exhaustive and new activities and ways of working will be required from all employees in order to ensure that the practice continues to provide patients with the highest standard of medical care and support services.

Salary dependent on experience.
