Job overview
The successful candidate will join the innovative specialist nursing speciality within a highly motivated Community Neurorehabilitation Service. The team consists of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Clinical Psychology and Nursing .They will also be working closely with the Consultant Neurologists from the specialist centre from Salford providing support for patients in primary and secondary care.
Main duties of the job
The role of the Community Parkinsons Specialist Nurse will be to offer high level clinical assessments and reviews to patients in a nurse led community clinic and in their own home, implementing a clinical care plan to support the patient in their home environment.
The post holder will demonstrate a high level of understanding of the condition and associated disorders and be able to translate that into practice.

Detailed job description and main responsibilities
The post holder will be expected to support prescribing and attend the nurse prescribing (NMP) course, acknowledging their own limitations as set out in the NMC code of professional conduct. Training and support will be provided as required.
It will be expected that the post holder will have a sound understanding of the community specialist nurse role and have a firm knowledge of utilisation of other allied health professionals within community services.
They will be expected to demonstrate that they can monitor and improve standards of care through audit and supervision, evidence based practice, learning and development.

If successful this vacancy will require an enhanced DBS check at a cost of £54.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
Person specification
Education & Qualifications
Essential criteria- Degree or Diploma in Nursing studies
- Evidence of continuing professional development including relevant attendance at postgraduate courses and conferences in nursing.
- Valid Registration with the Nursing and Midwifery Council.
- ENB 148 Neurosciences or equivalent course
- ENB 998 or equivalent.
- Non – Medical prescribing
- A degree to Bachelors level; or evidence of working toward a degree or Masters qualification.
Knowledge
Essential criteria- Advanced Clinical reasoning skills.
- Clinical audit.
- Understand the legal responsibilities of the profession.
- Knowledge of Parkinson’s and related conditions
- Risk assessment
- Up to date in professional practice and new research.
- Be familiar with the ‘Unified Parkinson’s Disease Ratings Scale’.
- Identify Parkinson’s as both a motor and non-motor disorder
- Awareness of NICE guidelines for Parkinson’s Disease management.
- Demonstrate essential knowledge on how Parkinson’s medication works, including knowledge of Dopamine dysregulation syndrome and neuroleptic malignant syndrome.
- Knowledge of intermediate care and community/support services.
- Knowledge of discharge planning processes.
Experience
Essential criteria- Relevant experience of working with Parkinson’s and associated conditions in either acute or community field.
- Previous experience in assessment of complex patients.
- Previous experience of people management.
- Experience as a mentor to staff and students on placements.
- Experience in dealing with patients in a palliative care situation.
- Experience of recognition of emerging cognitive impairment.
- Have significant experience in caring for patients with Parkinson’s or worked with patients with neurological conditions.
- Significant experience of mentorship and teaching.
