This is a exciting opportunity offering further development from a Band 3 Discharge coordinator to an established Senior Discharge Coordinator Band 4. We welcome applications from people looking for a developmental role or from individuals who hold relevant Band 4 skills and experience as described in the job description and person specification.

The individual will need to ensure careful discharge planning is considered from point of admission and all relevant and necessary processes are carried out that will facilitate discharge. All appropriate literature will need to be disseminated and understood by the person / carer or identified point of contact.

Role will entail recording those patients who are Clinically Ready for Discharge and a delay, considering the eligibility criteria and recording on inpatient clinical records.

To provide a single point of communication with Patients/Carers/Identified person to Service Partners/LA ensuring that the discharge processes are patient centred, consider the needs and wishes of the patient and those involved within their care.

To utilise IT/information within the Trust and partner agencies to facilitate improved patient flow enabling a better patient journey.

To ensure that discharge processes are followed and provide guidance when there is lack of clarity around discharge planning.

To ensure accurate and robust reporting is undertaken and shared with relevant care services.

When relevant and agreed by partners to undertake a trusted assessor role to support with patient discharges.
