Core Responsibilities Support the Infection Prevention and Control (IPC) lead in managing audits, providing training, supporting the team, and demonstrating compliance to the Partners. Collaborate with the Nurse Lead to produce the annual IPC statement.
Assist in developing, implementing, and integrating an effective care programme for frail and housebound patients registered at the Health Centre. Conduct patient assessments to ensure healthcare provision is appropriate and evidence-based. Create, implement, and embed health promotion and wellness programmes.
Develop and evaluate individual care plans for chronic disease patients, supporting their health and wellbeing. Identify, manage, and support patients at risk of developing long-term conditions to prevent negative health outcomes. Provide routine nursing care, based on clinical evidence, NICE, and NSF guidelines, as needed.
Diagnose and manage acute and chronic conditions as required. Prescribe medications within the scope of practice and Patient Group Directions (PGDs). Review patient medications to enhance adherence and support safe usage. Offer advice to patients on prescribed and over-the-counter medications, including dosage and potential side effects.
Coordinate travel medicine services. Request pathology services and handle pathology results as required. Oversee chronic disease clinics, providing necessary patient care and referring to secondary/specialist services as needed. Maintain accurate clinical records in compliance with legislation.
Support the Lead Nurse in ensuring effective use of Snomed codes across the nursing team. Oversee the maintenance and use of chronic disease registers. Participate in both well-woman and well-man clinics. Ensure adherence to the Health Centres chaperone policy.
Assist GPs with minor surgical procedures when necessary. Prioritise health concerns and intervene when appropriate. Support the team in managing clinical emergencies. Identify, assess, and refer patients with mental health needs. Administer vaccination programmes for both adults and children.
Support patients in understanding and managing their prescribed or over-the-counter medications. Collaborate with external services and agencies to ensure appropriate patient support, especially for vulnerable patients. Delegate clinical responsibilities appropriately, ensuring safety and adherence to Health Centre scope.
Support the clinical team in all safeguarding matters, following local and national policies. Understand and apply local and Health Centre's policies for substance misuse and addictive behaviors, referring patients when appropriate. Deliver health promotion opportunities as they arise.
Secondary Responsibilities Assist in developing and reviewing audit protocols and processes, utilising the audit cycle effectively. Collaborate with the partners and Lead Practice Nurse to develop, review, and implement nursing protocols. Support nursing team members, providing guidance, conducting appraisals, and identifying training needs.
Engage in local initiatives to enhance patient care and service delivery. Support shared learning within the Health Centre to improve patient outcomes. Regularly review clinical practices, responding to national policies and initiatives as appropriate.
Participate in significant and near-miss event reviews, using root cause analysis (RCA). Assist the Lead Practice Nurse in Coordinating the nursing rota to ensure adequate staffing to meet patient needs. Collaborate with the partners and Lead Practice Nurse to Lead planning and implementation of changes within the nursing team to improve service provision.
