A vacancy has become available at West Hertfordshire Teaching Hospital NHS Trust to join our team of Trauma Rehabilitation Co-ordinators.
This post offers the successful candidate the chance to work closely with medical, nursing, and therapy colleagues to coordinate effective pathways of care for Major Trauma patients in one of the busiest trauma units in the region.
We are seeking an experienced Band 6 or Band 7 Occupational Therapist, Physiotherapist, or Registered Nurse to join the team.
This is a part-time role (0.6 WTE), working three days a week, ideally towards the end of the week.
Weekend working on a Saturday is required as part of a rota, with a maximum of 1 in 4 Saturdays worked, offset by a rest day during the week.
Main duties of the job
You will be working closely with colleagues to identify Major Trauma patients and will be responsible for organising and managing the complex care and rehabilitation pathways for trauma patients.
You will support the team with complex trauma patients with treatment planning, joint clinical sessions and help case manage the patients journey through their hospital stay.
You will lead on coordinate their discharge, including ongoing care or facilitating transfer to an appropriate rehabilitation facility.
You will work collaboratively with the wider MDT to help coordinate patient pathways, including repatriation, complex discharge, referrals to complex rehabilitation and ensure timely completion of rehabilitation prescriptions.
You will act as a single point of contact for liaison with Major Trauma Centres and other Trauma Units and support repatriations, gaining thorough handover of patient status.
You will need to be confident and proactive in your communication skills and complex caseload management with advanced clinical reasoning skills to drive excellent patient care.
You will actively participate in Trauma Network meetings, participate in reviews and trust committees, lead local audit and develop and review procedures, guidelines and protocols.
You will be responsible for ensuring requirements as set out by TARN (Trauma and Rehabilitation Network) are being met through regular training and updates of the therapy and nursing team.

Person specification
Education and qualifications:
Essential criteria- Member of Health and Care Professions Council or Nursing and Midwifery Council.
- Health Care degree qualification or equivalent & NHS experience.
- Documented evidence of Continued Professional Development of expanded and highly specialised knowledge and skills.
- Attendance at post graduate courses in relevant fields (specific to the post).
- Broad range of clinical experience across acute and community care.
- Senior experience clinically in a range of clinical areas e.g. major trauma, trauma orthopaedics, neuro specialities, critical care.
- Mentorship/teaching/student experience or equivalent.
- Attained or working towards a Post Graduate Degree or relevant qualification.
- Membership of a professional Special Interest Group or National or Regional Network.
Knowledge:
Essential criteria- Understanding of organisational structure of the NHS and Social Care.
- Demonstrate an ability to provide professional/clinical leadership.
- Understanding of Standards – Health Care Professions Council/COT/BAOT/Local/National.
- Understanding policies and procedures (local e.g. Health and Safety).
- Demonstrate ability to search and review the evidence base.
- Demonstrate evidence of written projects/audit.
- Demonstrate understanding of clinical governance and the implications of the service.
- Specialist knowledge of conditions relating to traumatic injury.
Experience:
Essential criteria- Teaching: Staff, Patients, Relatives, Carers, Individually and in groups.
- Ability to teach and present to qualified/non-qualified staff and other disciplines, in groups or individually, using a variety of media.
- Ability to lead clinical education of less experienced staff.
- Experience of working within a multidisciplinary team.
- Demonstrate presentation skills in a variety of media.
- Clinical problem-solving ability.
- Experience of case co-ordination.
