Clinical Fellow in Paediatric Intensive Care Medicine:
The clinical fellow post in Paediatric Intensive Care Medicine provides training in all aspects of Paediatric Intensive Care Medicine. The PICU at St. Mary’s Hospital is recognised for training for Year 1 of the two year Grid training programme in Paediatric Intensive Care Medicine.
The PICU at St. Mary's has established itself as a major referral centre for children with life threatening medical illnesses. The Department of Paediatrics is developing, as its major clinical and research area, the field of Infectious Diseases.The unit has been designated the Regional Referral Centre for HIV infected children and is also a referral centre for infectious diseases including those requiring intensive care. It is also one of the four paediatric major trauma centres in London and a HCID referral centre for both airborne and contact HCID.
There are many research interests within the department and the PICU is proud of its research portfolio. Areas of current research include clinical trials including in non-invasive respiratory support, bronchiolitis care, artificial intelligence models for ventilation, endotyping of paediatric sepsis patients and point-of-care ultrasound in critical care.The unit is a specialist centre for high consequence infectious diseases.
Main duties of the job
Job1:Clinical Fellow in Paediatric Intensive Care Medicine
The fellow will be involved in the day to day running of the PICU/HDU at St Mary’s. While on duty, the fellow will at all times be expected to take responsibility for every aspect of intensive care, including airway and ventilation, vascular access and general management of newly admitted and existing patients on the PICU/HDU.The fellow will always remain under the supervision of one of the Consultant Intensivists. The fellow is expected to play a lead role in the PICU audit, including involvement in morbidity and mortality meetings, and other clinically based projects.
Job 2:Paediatric Intensive Care Clinical Research Fellow
One of the 3 posts offered will be for a ‘research fellow’ who will have 40% of their time specifically allocated to do research. This fellow will be involved in clinical research projects that are ongoing on the unit. There will be the opportunity to take a lead on specific research studies in the postholder’s area of interest, as well as to act as Associate PI for multicentre studies to gain experience in running clinical trials.It is expected the postholder will learn how to conduct clinical trials, how to apply for ethics permission, how to develop a research study and basic data analysis skills.
On call: The rota is a 15 person full shift rota, middle grade and in line with the New Resident Doctor Contract

Detailed job description and main responsibilities
General responsibilities
When working a long day or a night shift, Fellows are expected to focus on the management of the patients on the PICU. When working on an early shift, Fellows are expected to work with the long day team, when necessary. There are education weeks, where Fellows will be able to attend meetings, prepare presentations for PICU meetings, attend operating theatre lists and augment their training appropriate to their training needs which will be discussed with their educational supervisors.

Wednesday All day PICU teaching
Thursday 12.45-13.45 Major Trauma Meeting

Last Wednesday Every month – North Thames core curriculum training in PICM – all day meetings
Person specification
Education/ qualifications
Essential criteria- On GMC registered/ licensed Medical Practitioner
- MRCPCH or equivalent
- Higher degree such as MD, PhD, Masters degree in a relevant field.
Communication
Essential criteria- Good spoken and written English is essential
- • Must have the required grade in an occupational English test if required for GMC registration
Relevant experience in specialty/Sub specialty
Essential criteria- At least 4 years of post-graduate medical training to include at least 2 years of training in paediatrics, critical care or anaesthesia.
- Experience must include at least 6 months of training in a critical care area, either adult, paediatric or neonatal intensive care
- Experience of paediatric intensive care medicine, paediatric anaesthesia and paediatric intensive care retrieval medicine.
Clinical skills
Essential criteria- Paediatric intravenous cannulation
- Lumbar puncture
- Basic airway skills including bag valve mask ventilation
- Advanced airway management
- Chest drain insertion
- Central venous line insertion
Academic achievements including research/publications
Essential criteria- An active interest in research and evidence-based medicine can be demonstrated
- Presentations of research at National Meetings
- Evidence of relevant publications in peer reviewed journals
Language
Essential criteria- • Good spoken and written English is essential

Essential criteria- • Experience and enthusiasm for clinical governance, quality improvement and risk management at a local level with evidence of projects completed
