The Virtual Frailty Ward team is delivering a responsive, safe effective caring service for older people to receive acute care in their own home, to avoid hospital admission. The team is commissioned to reflect the needs of the local community.

Service aims include: same day emergency response with assessment of patient in ED/ at home: management of a caseload of inpatient and community based patients, reviews of self-care management and adoption of a co-produced approach with patients and service users.

The Virtual Frailty Ward operates from within the acute Trust, and hospital at home within the patients usual residence, delivering a joined up approach across the local Frimley Health and Care Integrated Care System (ICS), with the ambition to deliver high quality excellent care for people living with frailty.

The hospital at home service may operate from a base at Frimley Park or Farnham Hospital.

To provide in office administration of the Virtual Frailty Ward, Urgent Community Response Team & hospital at home team and the services they provide.

General administration
  • To provide administrative support for the Consultant Nurse for Older People, Hospital at Home Practitioners, Registered nurses, in-reach GPs and the Consultant Geriatrician team
  • To manage frailty team’s records and databases including:
  • Developing, using and maintaining databases and spreadsheets containing patient details and outcomes.
  • Responsibility for ensuring accuracy of patient records, updating them with new or corrected information as required
  • Scanning and uploading documents to various patient record systems as required
  • Sharing information/documentation with community partners as necessary, within data protection guidelines.
  • Producing and sending letters to GPs
  • Follow-up phone calls to patients who have been discharged from hospital
  • To act as a point of contact for the Hospital @ Home team & frailty team, responding sensitively to issues of a difficult nature and referring to other team members as appropriate
  • To deal with requests for patient information from internal staff and external organisations efficiently and in line with data protection guidelines
  • To contact via phone and/or email other care providers to obtain patient information in line with data protection guidelines
  • To monitor shared email inboxes and ensure referrals are received by the relevant team members in a timely manner
Team administration
  • To assist the Advanced Clinical Practitioner(s) with rota planning and maintenance
  • To deal with requests for administrative support from the Virtual Frailty ward team / Hospital at home team, UCR Practitioners, in-reach GPs and the Consultant Geriatrician team, including but not limited to requests for uniform, ID badges, training, etc
  • To accurately maintain other relevant team documentation as required
  • To undertake other routine administrative tasks as required (e.g. note taking, formal minute taking, meeting room booking, document scanning, filing, document formatting, photocopying, etc)
  • To attend relevant meetings as required
  • To be responsible for the ordering and maintenance of the team’s stationery supplies.
