This is an exciting time as we make changes to the way we deliver treatment to enhance the care for our patients who are living with and beyond cancer through direct on-site support, which will enhance patient experience, promote continuity, strengthen communication, and enable timely assessment and review of individual needs.
The potholder will work as an autonomous practitioner and provide direct support to patients receiving NSO out-patient care away from their usual, local hospital identifying and addressing any supportive care needs.
The team are based at Weston Park Cancer Centre, but you will be expected to work flexible across South Yorkshire and Bassetlaw therefore must be a car driver/owner with a full driving licence valid for the UK. The successful registered nurse will require substantial post registration experience and must be able to demonstrate evidence of working with patients living with and beyond cancer.

Main duties of the job- To work as an autonomous practitioner and provide direct support to patients receiving Non-surgical Oncology (NSO) out-patient care away from their usual, local hospital.
- Be responsible for supporting the patient whilst attending their NSO out-patient appointment, identifying and addressing any supportive care needs and communicating these effectively back to the Trust CNS team in order that care is seamless.
- Provide input and care to patients from each of the respective 4 Trusts outside the tertiary centre (Barnsley, Rotherham, Doncaster, and Chesterfield) and become a regular point of contact for patients attending, enabling greater continuity by acting as a conduit between the patient and their site-specific CNS team.
- Operate as cancer experts with the ability to assess and respond to complex patient needs referring site-specific issues to the relevant site-specific CNS team.
- These roles will primarily focus on ensuring patients receive high quality, personalised care, and support. Post-holders will help to shape and strengthen a cross-system support framework involving key stakeholders across a range of teams, organisations, and sectors.
- To address known inequalities across NSO pathways and participate in the delivery of remedial actions to address.
- Improve NSO pathway timeliness and coordination through greater oversight to promote a positive patient experience and greater compliance with national Cancer Waiting Times standards.

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being externally funded. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the ongoing source of the external funding and the requirement for the post to be maintained.
