This is for a support worker in a mental health Supported living home. The service in Hyde Park supports people with severe and enduring mental health issues.
Shifts include working weekends and doing nights.

Main duties of the job
The role is to support people who live in the home with daily life skills and activities, Updating support plans and risk assessments. Updating daily notes and ordering, booking in and supporting clients with medication and general housing keeping of the service.
Carrying out Health and Safety checks daily and completing records.
Working with other professionals involved in the clients care. and families.
You will also support clients to health care appointments as required and to access the community
