Northampton - Community Psychiatric Nurse - Memory Assessment Service
Job overview
The post holder will work as part of a specialist Memory Assessment Service (MAS) to provide comprehensive assessment, diagnosis, information and support, whilst working with others within the multi-disciplinary team and external stakeholders to provide the best outcomes for service user’s and carers, and managing a caseload.
The post holder will act as Care Programme Coordinator (CPA) in the community with individuals (and their support networks) who have complex mental health problems.To assess needs, plan, implement and evaluate treatment packages in line with service policies providing comprehensive care packages to those individuals who are
experiencing problems with mental health.
Main duties of the job
To undertake specialist memory assessment for referred service users in a range of settings across the catchment area (clinic settings, service users own home, via the internet, telephone).
To deliver a range of treatment programmes and/or interventions to support recovery, to include clinical duties and interventions.
To provide structured therapeutic interventions with potentially complex clients in one-to-one session or group settings as appropriate.
To manage a caseload and to prioritise effectively.
To participate in multi-disciplinary team working to ensure service user’s needs are met

NMC
Essential criteria- NMC registration in Mental Health Nursing
- Completed assessor/supervisor training or willing to complete
Knowledge and Experience
Essential criteria- Able to work autonomously and as part of a team
- Experience in working within the NHS
- Knowledge of SystmOne
Communication
Essential criteria- Excellent communication skills evidenced within the application
Skills
Essential criteria- Self-motivated and able to work effectively under pressure
