To be responsible for completing the medicine reconciliation process, assessing Patients Own Drugs (PODs), transcribing, and ordering of medicines as an accredited medicines optimisation pharmacy technician when necessary.
To support the implementation of self-administration by patients and/or carers where possible. To support the management of the medication supply pathway including the efficient transfer of medicines between day units and wards. Participate with patient counselling by undertaking specific accreditations in order to carry out counselling for oral chemotherapy.
To liaise with ward and pharmacy staff to resolve any technical queries or problems with patients, referring to ward pharmacist when required. To liaise on drug related issues with primary healthcare professionals, as appropriate for patients being discharged from hospital.
To participate in clinical audit and data collection including medicines safe & secure or other forms of research and development in collaboration with the sister/nurse in charge. To complete the wards/units quarterly Controlled Drugs (CDs) audit, including balance transfers to new CD registers and checking of CD register balance with the physical stock, reporting any discrepancies in a timely manner.
To be involved in the training of new clinical pharmacy ward staff, pre-registration trainees, undergraduates, and peer reviews of existing medicine MMTs. To support the patient medicine pathway by working closely with the pharmacy aseptic unit staff, stores and procurement, clinical trials and all other relevant pharmacy teams and act as a link between the day units and these pharmacy areas, ensuring that a seamless high-quality service is provided.
To undergo practice/educational supervisor training programme to provide supervision for less experienced staff and trainees. To participate with all activities within the dispensary when required. To work as an Accredited Checking Pharmacy Technician (ACPT) to undertake final accuracy checks on dispensed medicinal products.
Assist in the directing, supervising, and coordinating of the daily running of the dispensary according to standard operating procedures when necessary. In conjunction with the Chief pharmacy technician clinical services support with the review, development and implementation of operational procedures and policies.
Participate in reviews of work methods and contribute to the development of new processes and systems to minimise risk, and to ensure that services are improved continuously. To actively report incidents/near misses, to mitigate future risks and when appropriate to support in the investigation of incidents, identify root cause and to establish plans to mitigate future risks, in a timely manner.
Assist with the recruitment and selection of pharmacy technicians and pharmacy assistants when required. To undertake Continuing Professional Development (CPD) to ensure a high level of competency in carrying out the tasks required and when appropriate to facilitate the CPD of other pharmacy staff.
To commit to flexible working patterns (late evenings, weekends & bank holidays) when required to meet the needs of the patients and service. To carry out any other reasonable duty as requested. Receive prescriptions from in and outpatient areas from nurses, HCAs, etc, checking the email prescription inbox twice daily and actioning the requests communicating with the OCCYP Daycare pharmacist and CYP medical/ANP prescribers to ensure completion of requests in a timely fashion.

Provide education and training for staff within the OCCYP Daycare and Outpatient services, to ensure appropriate counselling and knowledge of processes in the absence of pharmacy technician. Supporting the team by arranging education where needs identified in collaboration with the daycare pharmacist.
