Duties and Responsibilities Communication and Working Relationship Skills Management Supervision will be provided by the Locality MDT Clinical Team Lead Supervision will be recorded on the Devon Partnership NHS Trust web based Supervision recording system.
Performance monitoring will be undertaken by the Locality MDT Clinical Team Lead. To support the multi-disciplinary team approach around an individual including attending appropriate meetings. Work in partnership with all relevant agencies. To work in a person centred way. Communicate effectively with clients, carers, team members and all relevant agencies.
Assessment of client / carer communication needs. Use of relevant forms of communication, e.g. written, verbal, e-mail, sign language and total communication. Communication skills including empathy, listening, reflecting and building trust and respect.
Awareness of difficulties when communicating with client who may not understand. Contribute to the work of specific project groups within the CCGs and across the local health community as required. Planning and Organisational Skills Promote effective public, user and carer involvement in all elements of work.
Responsibility for Patient/Client Care, Treatment and Therapy To be responsible for the clinical assessment of needs, planning of individualised appropriate care, implementation and evaluation of care in conjunction with appropriate primary health services.
Effective care management of cases caseload to be agreed by Team Manager Carry out assessments as appropriate for new referrals, including health assessments for Continuing Health Care. To provide observation and support around Learning Disability issues in a variety of community settings with clients who have complex health needs.
To support people with a Learning Disability to access mainstream health services (both primary and secondary health.) To liaise with mainstream services around Learning Disability issues linked into specific clinical tasks. Promote health and independence of the individual.
Have a clear understanding of the Safeguarding Adults legislation and participate in investigations as required and directed by Team Manager. To promote the use and development of Health Action Plans to improve an individuals understanding of their health and how to keep well using a person centred approach.
To provide support to carers as appropriate having an awareness of the carers health. Adhere to confidentiality policy. To undertake clinical risk assessments. To support the multidisciplinary team approach around an individual including attending appropriate meetings.
Responsibility for Policy and Service Development Implementation Ensure that their clinical practice is based on evidence and good practice. Have a clear understanding around issues of consent and capacity. Be aware of clinical governance issues. Contribute to nursing practice development.
Keep up to date with developments in Learning Disability nursing. Contribute to the provision of a learning environment. Attend training and develop competencies in line with service demands. To contribute to service development. To take part in clinical audit and service governance.
Contribute to Learning Disability Partnership Strategies around implementing Valuing People, Now, Six Lives and Meeting Health Needs of People with Learning Disabilities. Contribute to the implementation of key Devon Partnership Trust issues.
Contribute to the work of specific project groups within the CCGs and across the local health community as required. The post holder will take responsibility for their ongoing personal and professional development through the Personal Development Review process in dialogue with their line manager.
Responsibility for Finance, Equipment and Other Resources Ensure security and maintenance of equipment. Responsibility for Human Resources, e.g. Supervision, Training, HR Advice and Management Receive clinical and management supervision. Monitor Community support workers whilst they are lone working.
Take a lead role in the development of the team Action for Health Agenda, in conjunction with the operational management group. Mentoring of student nurses and staff from other agencies. Responsibility for Information Resources and Administrative Duties To provide up to date reports / assessments and present at meetings / reviews / Safeguarding Adults strategy meetings and social care funding panels.
Keep accurate, factual and contemporaneous nursing records and other relevant paperwork in line with NMC guidelines and Trust / team policies. Input data onto clinical recording systems.
