The Healthcare assistants will work as part of a multidisciplinary health and social care team. The Healthcare assistant will work at a level above the health care assistants and have additional skills to support management plans implemented by registered health and allied health care professionals.

JOB PURPOSE The Healthcare assistant will assist in the identification of health needs and participate in the delivery and evaluation of care that meets the needs of a designated patient caseload. Working in partnership with a range of health care professionals they will undertake a range of delegated tasks that include Supporting the admission/ discharge of patients Facilitating rehabilitation programs Clinical interventions Medications management Patient Centred Care Provide excellent customer care skills to ensure patients come first.

Ensure that patients receive high quality clinical and personal care and a good patient experience, having regard for their customs, religious beliefs and doctrines. Recognise the patients right to privacy, dignity and confidentiality, showing due regard for the patients property.

Provide support and assistance to professionals in the implementation of plans of care for individual patients. As directed, collect specimens from patients and to record routine observations of weight, blood pressure, temperature, pulse, respiratory rate.
Report any abnormalities and concerns to a Registered Nurse. Assist with washing, bathing, showering, hair and oral care, dressing and undressing. Recognise when patients are symptoms or general wellbeing has changed and report to nurse in charge.
Monitor and report pressure area concerns Under the direction of trained staff, maintain and develop own skills, including: monitoring and recording of vital signs to include blood pressure, monitoring and recording of blood glucose levels, oxygen saturations.
Be responsible for the management of delegated wound care, undertaking dressings using an aseptic technique ensuring appropriate documentation is completed and updated and any variance reported to the registered health care professional Assist in the management of continence undertaking delegated duties to catheterise patients following assessment by a registered health care professional (Bladder scans), catheter care drainage and stoma care and incontinence garments.
Respond appropriately to clinical emergencies to ensure the patient receives appropriate care. Ensure the safe handling and mobilisation of patients and when necessary to assist in the transportation of patients. Chaperone patients to and from home / hospital / hospice, following assessment by a registered nurse, to maintain patient safety.
Care for the body after death and support family members / carers appropriately Respond to patient requests for non-clinical information to improve communication. Provide comfort and support to patients, carers and family. Medications Management Administer medication under the direction of an Registered health care professional and in accordance with Hospice policy and procedure.
Monitor the administration of medication via syringe drivers and undertake first line troubleshooting when problems occur Complete all relevant documentation appropriate to medication and administration including syringe driver monitoring sheet Caseload Management for designated cohort of patients Support the transfer of information via team leader, MDT and ward rounds regarding patients progress.

Attend and actively participate in care planning meetings, case conferences and MDT Meetings Undertake referrals to appropriate health care professionals Ensure all essential information is collated on EMIS for your designated cohort of patients, updating this information as patients choices and preferences change Record a summary of care at agreed designated times Extended roles- Rehabilitation Demonstrate knowledge and skill to use and teach others how to use a range of mobility and therapeutic equipment including wheel chairs, standing frames, hoists and exercise programs Take responsibility for the organisation and planning of designated patients in your caseload group, including basic evaluation and progression of treatments, reporting changes and responses to treatments in a verbal and written format to the qualified staff Facilitate individual treatment plans under the direction of the physiotherapist Monitor and report progress Complete relevant assessments chart Undertake falls risk assessment reviews and complete relevant documentation and fall risk warnings CLINICAL SKILLS Physio and rehabilitation Vital signs monitoring Wound care Individual patient risk assessment and review Clinical skills- catheterisation Medications management to include subcutaneous injections GENERAL RESPONSIBILITIES To undertake all mandatory training as required by Trinity Hospice and Palliative Care Services and participate in appropriate in-service training as and when required.

Maintaining the strict confidentiality of all information acquired especially with regard to patients and staff. To ensure all documentation is contemporaneous, clear and accurate, and plans of care are updated in accordance with the needs of the patient.

To undertake an appraisal and personal development review annually and through self-development, continuously update and improve knowledge and competencies.
