Job overview
To work under the guidance and supervision of qualified members of staff to meet the needs of the clients in their own home. The job may also occasionally require the provision of care of clients in some other primary health care setting. Calderdale provides a 24 hours district nursing service and the job entails supporting this service.
The post is for an out of hours healthcare assistant working between the hours of 18:00 - 08:00 (depending on shifts) 7 days a week, based at Allan House Clinic Sowerby Bridge.- A valid UK driving licence is essential for the post with access to your own vehicle *
Main duties of the job
Applicants must be a motivated and passionate about high quality patient care have excellent interpersonal skills, be flexible and have an ability to work under pressure. Assist in the delivery of unscheduled, unpredictable episodes of care on a regular basis, with appropriate use of initiative.
Be willing to undertaken any additional training appropriate for this role, to enable effective delivery of care of the client group.
Previous experience in a healthcare setting is desirable.
It is essential that the candidate is confident , resilient and has a caring nature.

Detailed job description and main responsibilities
Able to create a professional nursing culture which is open to change, new ideas, concepts and innovation whilst reflecting organisational strategy and values.
Willing to develop new skills and participate in training in line with servicedevelopment e.g. venepuncture, administration of defined medicines, enteral feeding.
Has an understanding and knowledge of community care procedure and practice (basic observations, catheter care, monitoring of blood sugar/blood pressure etc)
Participate, through appraisal, in matching organizational aims with personal objectives and supporting staff within their own area of responsibility to do the same.
Managing Information: Use appropriate information to make timely and effective decisions in relation to the community nursing team.
Contribute to the development of information systems.
Obtains transmits and stores information relating to the delivery of care services (this may involve auditing)
Participate in the use of information and technology including inputting data on the computer system enabling other members of the team to use.Provide information in a form that meets the understanding of the receiver at all levels.
Use of oral, written and computerized information that facilitates continuity of patient care throughout the 24-hour period.
Completes documentation in accordance with Trust’s policies and procedures
Managing Self: Participate in regular supervision.
Attend all mandatory training.Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.
Comply with all Trust policies, procedures and protocols.
Carry out duties with due regard to the Trust’s Equal Opportunity Policy.
Seek advice and support from Line Manager whenever necessary.
Maintain professional conduct including appearance at all times.
Ensure maintenance of Professional Registration.

QUALIFICATIONS / TRAINING
Essential criteria- English and Math GCSE grade 9-2 (A*-E) or equivalent or high level of literacy and numeracy skills demonstrated within application
- Competence based level 2 Qualification e.g. NVQ or Diploma in Care or The Care Certificate issued by an employer
- Willingness to undertake any training or development relevant to the role
- Competence based level 3 Qualification e.g. NVQ or Diploma in Healthcare Support
KNOWLEDGE, EXPERIENCE & EXPERTISE
Essential criteria- Previous employment with a healthcare setting (18 months)
- Insight into the roles of responsibilities of the post
- Demonstrate a commitment to a caring profession
- Understand the importance of record keeping
- Literacy Skills
- IT skills
- Car owner/driver or suitable alternative transport to enable you to undertake the job (it is unlikely that public transport will meet this requirement). Reasonable adjustments can be considered in accordance to the Equality Act

The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £54.40, Standard £26.40 or Basic £26.40) from your salary when you commence in post (including Internal staff).You will also be required to participate in the DBS Update Service and pay the £16 cost per year. This is a condition of your employment.
