CPN/Care Coordinator

Please refer to the attached job description and person specification for full details of responsibilities. The post holder will hold a case load of patients suffering with severe and ensuring mental illness, working towards the recovery model to support patients to achieve positive outcomes.
The post holder will provide a good role model, develop and maintain good systems of communication, be able to communicate with a wide range of people in a range of situations. Reporting verbally and in writing with regards to clients progress, assessments and care planning to the wider multi-disciplinary team.
Maintain accurate and complete records. Discussing with client and their families / carers regarding care planning, assessments and treatment. Be able to acknowledge the barriers to effective communication and adapt communication style in response.
Maintain professional standards in line with NMC code of conduct and Trust Nursing Strategy. The post holder will monitor and maintain the health and security of self and others. Managing identified risks. Ensuring that health and safety legislation is adhered to.
Manage emerging situations, taking action as appropriate. Reporting actual and potential incidents. Attend Trust mandatory training. The post holder will contribute to the continuous improvement of services. Identifying the implications of direction, policy and strategy on current practice in a constructive and timely manner.
