The Rotherham Assertive Outreach Team (AOT) provides intensive, recovery-focused
community support to adults across Rotherham with severe and enduring mental illnesses, primarily psychosis. The service works with patients who often present with co-existing
substance misuse, complex risks, and significant social challenges, many of whom have

struggled to engage with traditional mental health services. AOT aims to reduce barriers to

care, prevent hospital admissions, and promote rehabilitation through a highly integrated, multidisciplinary, and multi-agency ‘team’ approach in line with the AOT model. The service

also includes a community rehabilitation pathway for patients with both psychotic and non
psychotic disorders (SMI), and a 10-bedded supported living facility offering structured, step-down rehabilitative support in partnership with South Yorkshire Housing.
The post-holder will deliver assertive and therapeutic interventions aimed at reducing
relapse and hospital admissions. Using DIALOG/DIALOG+ and other patients reported
outcome measures, the Care Coordinator collaboratively assesses, develops and reviews
personalised care and recovery plans, ensuring that support is holistic, person-centred, and

delivered in partnership with the wider multidisciplinary team and external agencies.

Main duties of the job

Deliver community-based assertive outreach and rehabilitation supportive engagement, promoting recovery, independence, and social inclusion.
  • Manage a defined caseload of patients in the community and supported living, including patients subject to Community Treatment Orders and act as Social Supervisor for patients subject to Ministry of Justice sections.
  • Act as shift coordinator, planning daily visits and care delivery, responding to service demand and crisis situations.
  • Work collaboratively with the multidisciplinary team to manage complex risks. Work collaboratively with the team, patients, carers, and external agencies, including South Yorkshire Housing to deliver holistic care.
  • Apply professional judgment in clinical decision-making, risk assessment, and care planning within Trust and statutory guidance.
  • Deliver care and treatment for patients based on individualised care plans and risk

assessments.

Qualifications

Essential criteria
  • NMC or relevant allied health professional registration; Qualified mentor/practice assessor
Desirable criteria
  • DESIRABLE Post-qualifications, courses in relevant subjects (e.g., Voice Hearing Maastricht Training, DBT skills, Phlebotomy, ECG)

Experience

Essential criteria
  • Experience working with adults with SMI, psychosis, Bipolar
  • Experience of completing complex risk assessments
  • Experience of working in a multidisciplinary team
Desirable criteria
  • Experience of shift coordination and managing daily care delivery.
  • Experience of working in an Assertive Outreach Team.
  • Experience of working in a community rehabilitation setting.
  • Experience of working with families and carers.
  • Experience of working as a Social Supervisor.

Knowledge

Essential criteria
  • Knowledge of the Mental Health Act, Mental Capacity Act, Safeguarding, CTOs and Ministry of Justice sections, recovery-focused interventions, risk management, team-based approach, crisis response
Desirable criteria
  • Knowledge of AOT ethos and principles, relapse prevention

Skills

Essential criteria
  • Leadership skills with the ability to support and supervise junior staff
  • Excellent interpersonal and communication skills; IT competency (Word, Outlook, Excel, PowerPoint); prioritisation; working under pressure; using PROMs; personable, adaptive engagement skills; ability to complete tribunal reports and shift coordinate.
