Description
We are seeking a dedicated and compassionate Case Worker WCR 3 Max to join our team. In this role, you will be tasked with providing comprehensive support to clients facing various challenges. Your primary responsibilities will include conducting assessments, developing personalized care plans, and ensuring that clients have access to essential resources.You will collaborate with diverse community organizations and service providers to address the individual needs of your clients, enabling them to achieve their goals and improve their overall well-being. This position requires strong interpersonal skills, empathy, and the ability to handle complex situations with professionalism and sensitivity.
You will also be responsible for documenting case notes and maintaining accurate records as mandated by our organizational policies. The ideal candidate will have a passion for helping others and a commitment to advocating for clients' rights and needs.

ResponsibilitiesConduct initial and ongoing assessments to evaluate client needs and strengths.
Develop and implement individualized care plans in collaboration with clients and other service providers.
Monitor client progress and adjust care plans as necessary to ensure effective support.
Link clients to community resources, services, and support networks to facilitate their growth and recovery.
Maintain comprehensive case records in compliance with agency policies and regulatory requirements.
Advocate for clients' rights and access to resources, representing their interests in various settings.
Participate in team meetings and case conferences to coordinate client services and share insights.
RequirementsBachelor's degree in social work, psychology, or a related field; Master's degree preferred.
Minimum of two years of experience working in case management or social services.
Strong understanding of community resources and support services available to clients.
Excellent communication and interpersonal skills to build rapport with clients and collaborate with professionals.
Ability to manage a diverse caseload and prioritize tasks effectively.
Demonstrated empathy, patience, and a non-judgmental attitude towards clients.
Strong organisational skills and attention to detail in maintaining case documentation.
Hours Per Week: 36.00
Start Time: 09:00End Time: 17:00
Pay Per Hour: £24.05
