Staff Nurse – Band 5

The post holder will be responsible for the delivery of high quality care to patients with orthopaedic problems or fractures within the acute hospital outpatient setting. They are responsible for the assessment, planning, delivery and evaluation of evidence based individualized patient care within the boundaries of their role.

The post holder will contribute to the continuing development of standards of care including developments of nursing standards and monitoring of outcomes. The post holder will be expected to continue with their own professional development.

Main duties of the job

The post holder will be responsible for providing excellent, evidenced based patient care with-in the Treatment Room, Plaster Room and outpatient clinic setting. An interest in tissue viability, wound care would be useful.

The post holder will provide support and act as a knowledge base for Health Care Support workers and student Nurses and Band 4 Nurse Associates.

High standards of record keeping are expected including photography of wounds.

The post holder will be able to run a Fracture Clinic, preparing patients for examination by removing dressings, plaster casts and splints. Act as a patient advocate when necessary.

Liaise with the MDT within the department and wider community.

Detailed job description and main responsibilities
  1. Plan, deliver and evaluate compassionate nursing care to meet the patients individualised needs.
  2. Ensure the highest standards of care and safety are maintained.
  3. To support the nurse in charge with the day to day planning, the delivery of a high quality nursing service and the continuous improvement of that service.
  4. To ensure effective communication is achieved and maintained of sensitive information, with regard to the patient assessments and clinical details.
  5. To act as a role model, ensuring high quality, professional standards are maintained and delivered at all times.
  6. To document observations, assessments, results, decisions and actions. Effectively in patient’s notes and communicate these effectively to appropriate members of the multidisciplinary team and as appropriate, to carers.
  7. Provide legible and comprehensive patient records to agreed Trust and NMC standards and guidelines
  8. To develop and maintain effective communication links with all levels of staff within the Trust and external agencies.
  9. To participate in the audit cycle of clinical practices and procedures including infection prevention & control initiatives.
  10. Assist in the monitoring and protecting of groups and individuals whose health and well-being may be at risk e.g. vulnerable adults.
  11. Promote and maintain the dignity and privacy of patients at all times.
  12. To participate and contribute appropriately in research, service modernisation, clinical governance and National Service Frameworks.
  13. To be responsible for developing and sustaining own knowledge, clinical skills and professional awareness in accordance with re-evaluation requirements and to maintain a professional portfolio with evidence of reflective practice.
  14. Actively encourage service users & carers for feedback and work with the outpatient team to develop action plans to address issues.
  15. To promote best practice and monitor support workers practice.
  16. To assist in the orientation and support of new staff and students to the unit, working together as a team.
  17. To act as a role model and promote the Trust’s STAR values.

Person specification

Qualifications

Essential criteria
  • First Level Registered Nurse
  • Current/live registration with the Nursing Midwifery Council.
  • Evidence of continuing professional development.
Desirable criteria
  • Qualification or experience in teaching and assessing.
  • Teaching/Assessor Qualification e.g. ENB 998 or equivalent.
  • Leadership qualification

Experience

Essential criteria
  • Relevant clinical and management experience as band 5 Staff Nurse In the fields of elective surgery or anaesthetics
  • Ability to organise work for caseload of patients
  • Prioritising clinical care needs
  • Nursing patients in an elective clinical setting
Desirable criteria
  • Previous experience of PreAssessment clinics
  • Previous theatre or outpatient experience
  • Working knowledge of computers

Knowledge

Essential criteria
  • Ability to supervise unregistered staff and students
  • Understanding of nursing standards and audit
  • Understanding of the principals and benefits of Pre-Assessment
Desirable criteria
  • Interested in undertaking research and audit
  • Willingness to undertake physical assessment and clinical reasoning course.
