Clinical Nurse / Pharmacist Specialist (NMP) ADHD Service - NHS

Main Responsibilities: To use evidence-based practice to develop, deliver and maintain a high quality, clinical and cost-effective ADHD service. To provide a comprehensive service to the patients attending the appointment including review of observations and investigations as needed, management, and referral as appropriate within defined parameters and scope of practice, referring to senior colleagues as needed.
Complex cases should be discussed with the Medical Lead, Dr S Mashru, including any titration that needs more than 3 follow-ups. To ensure patients have consented to, and are fully involved in the implementation of their treatment plans To implement Non-Medical Prescribing in accordance with NICE, NHS England and Trust prescribing guidelines, ensuring you work within your NMC / GPhC Code of Conduct / Scope of Practice.
You will be made aware of NWL ICB Prescribing Formulary for guidance around medications that are allowed to prescribe. You will be expected to sign the Harrow Health Intention to Prescribe Agreement with the Head of Nursing To adapt styles of communication appropriately to promote empowerment, self- management and health education for patients.
Interpreters can be sought as needed. Using the EMIS system, the consultation must be contemporaneously recorded in the patients records. Proformas are set on the EMIS system and must be completed by the clinician. These will be shared with the patient and their registered General Practitioner.

Liaise with other team members as appropriate To identify actual or potential clinical risks within scope of practice and take appropriate action using, when required, the appropriate reporting structure To be personally accountable for professional and ethical actions and practices within a framework that adheres to their governing body and Harrow Health policies and procedures Service management / delivery responsibilities: Ensure your caseload is effectively managed and systems are in place for follow-up review and discharge.

To ensure that patient records and documentation are maintained in line with Harrow Health policies and use of the electronic patient records system. This includes: Documenting full, accurate and contemporaneous records of assessments, investigations, procedures, treatments and aftercare.
Encouraging colleagues to maintain good and complete records. Working closely with the reception team / administrator, ensuring all records pertinent to patients are filed as appropriate Work within local policies, protocols and guidelines Harrow Health and local trust guidelines Ensure Safeguarding Adults and Children is considered, and policies and procedures are followed when concerns are raised and attend local safeguarding meetings as required.
Establish quality standards relating to the service user and carer experience. Ensure that service users and their carers receive clear and accurate information on the service, access to their records, confidentiality, personal care plans, medications, support services available and complaints and compliment procedures.
To participate in audits as requested, and use the results, current research, and government guidelines to make recommendations for changes to service delivery and clinical practice, sharing these findings as appropriate. To collect data, and provide service information to the Team Leader as required To ensure that the Clinical Manager / Team Leader / Head of Nursing and Clinical Governance Lead are kept informed on service issues and potential risks.
To work with the Team in delivering and promoting the needs of the service To work with the Clinical Lead, Team Leader, Head of Nursing and Clinical Governance Lead to meet the Clinical Governance needs of the services, including investigation of incidents (including serious incidents) and complaints, where required.
To participate in relevant team meetings to discuss day-to-day operational issues as required. Personal & professional development: To be a skilled practitioner in areas needed for effective care delivery o Recognise and work within own competence and keep abreast of developments in practice and topical issues ensuring that skills are maintained To collaborate in the continuing development of their role / expertise to include: - o Level of autonomy in practice.
o Show self-motivation toward expanding practice and developing skills and knowledge, with the support of Harrow Health Ensure statutory and mandatory training is up to date Promote a learning environment and culture within the team which supports the professional development and effectiveness of the team as a whole Contribute to the delivery of specialist and multidisciplinary training, including the induction of new staff and trainees within the service, if applicable.
